Main duties of the job

The role:

  • This is a specific role within the wider SE CRHTT to provide in-reach and support to our new Crisis House in Buntingford. Working with the wider CRHTT colleagues and professions the post-holder will predominantly work 5 days per week and in-reach as needed to support service users staying at the crisis House and our partner Agency - Waythrough - who are specialists in MH support in community settings.
  • Providing urgent assessments to those considered to have acute mental health concerns and to robust home treatment options as an alternative to hospital admission.
  • A role that is varied and challenging, offering an opportunity to work in a variety of settings, with people with a variety of mental health needs.
  • As gatekeepers for hospital admission, you will work as part of a multi-disciplinary team to provide safe and effective treatment in the community, and facilitate early discharges from the acute inpatient units.
  • Working collaboratively with our service users, their carers and our colleagues to positively manage risk in the acute care pathway, whilst enabling an individuals’ recovery journey.

Detailed job description and main responsibilities

  • Manage the referrals process for the Crisis House and undertake in-reach on a daily basis with Se CRHTT colleagues.
  • Work with an experienced clinician working with service users at various phases of their mental health.
  • Be passionate about making a difference for individuals and their carer’s during their personal journey.
  • Provide urgent assessments to those considered to have acute mental health concerns and to robust home treatment options as an alternative to hospital
  • Act as a gatekeeper for hospital admission
  • Work as part of a multi-disciplinary team to provide safe and effective treatment in the community, and facilitate early discharges from the acute inpatient units
  • Work collaboratively with our service users, their carer’s and our colleagues to positively manage risk in the acute care pathway, whilst enabling and empowering an individuals’ recovery journey
  • You must hold a full valid driving licence and have access to a car to use regularly for business purposes (unless you have a disability as defined by the Equality Act 2010)

Person specification

QUALIFICATIONS, EDUCATION, TRAINING

Essential criteria
  • RMN or Dip / BSc (Hons) in Mental Health Nursing
Desirable criteria
  • ENB 998 / Preceptorship and Mentorship. ENB 812 / BSc (Hons) Specialist Community Nursing Practice (Mental Health) or similar training.

PREVIOUS EXPERIENCE

Essential criteria
  • Post-registration experience with at least some experience at band 6
  • Experience in acute and community areas.
  • Experience in care planning, change management and use of Microsoft Office
  • Evidence of continuing professional development.
  • Knowledge of physical health contributions to Mental Health issues, difficulties and assessment
Desirable criteria
  • Experience in working with acute mental health problems in community setting
