Job overview

To work as part of the Integrated Community overnight nursing team to prevent inappropriate hospital admissions and support patients out of hours  The post holder will work collaboratively to deliver a high standard of patient care using an evidence-based framework to support patients with complex needs.

The post holder will support and manage the overnight team 

Main duties of the job

  • Support the operational management and day-to-day running of the team.
  • Assess patient understanding, gain valid informed consent and engage with significant others to work within a legal framework with patients who lack capacity to consent to treatment.
  • Ensure a high standard of clinical care for the patients under the care of HCT
  • Be responsible for the accurate maintenance of patient records in line with HCT Policy.
  • Develop and provide advanced clinical leadership and education to all clinicians and support staff working in the community setting
  • Support the induction of new staff and the supervision, education and assessment of other clinicians.
  • Ensure safe clinical practice and use of equipment by others through teaching, training and supervision of practice.
  • Identify areas for development and implement/be involved in initiatives which lead to changes in service provision which benefit our community.
  • Communicate effectively, sensitively and professionally with patients/carers/colleagues in a non-judgemental form that is appropriate to them and the situation including the exchange of factual information, using advanced communication techniques which respect barriers and patient’s dignity, wishes and beliefs.
  • Build relationships across professional and organisational boundaries, using influencing and negotiating skills to coordinate with others to ensure our patients receive care that respects their wishes.

Detailed job description and main responsibilities

·         If appropriate to the role/profession, practice as an independent Non-Medical Prescriber within current HCT policies/guidelines or have a willingness to undertake this as part of the role.

  • Develop and provide advanced clinical leadership and education to all clinicians and support staff working in the community setting
  • Support the induction of new staff and the supervision, education and assessment of other clinicians.
  • Ensure safe clinical practice and use of equipment by others through teaching, training and supervision of practice.
  • Take a lead role in audit, research and Quality Assurance activities in line with HCT Clinical Quality Strategy.
  • Identify areas for development and implement/be involved in initiatives which lead to changes in service provision which benefit our community.
  • Communicate effectively, sensitively and professionally with patients/carers/colleagues in a non-judgemental form that is appropriate to them and the situation including the exchange of factual information, using advanced communication techniques which respect barriers and patient’s dignity, wishes and beliefs.
  • Build relationships across professional and organisational boundaries, using influencing and negotiating skills to coordinate with others to ensure our patients receive care that respects their wishes.
  • Support HCT recruitment activities of other clinicians that wish to work in the community.

Person specification

Qualifications

Essential criteria
  • Registered Nurse
  • Mentor/Preceptor/998 or equivalent
  • Specialist Nursing Practice (District Nursing) Degree or will undertake training
  • Nurse Prescribing or will undertake training
  • Evidence of personal development

Experience

Essential criteria
  • Evidence of experience at band 6 Community Nurse level
  • Management of a team
  • Management of change within a complex environment
  • Experience of teaching others

Skills

Essential criteria
  • Communicating within a multi discipline team
  • Excellent oral and written communication skills
  • Management of change
  • Knowledge of clinical governance and its implications

Other requirements

Essential criteria
  • Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
