Stroke Occupational Therapist

Band 6

Job overview

Are you ready to make a real difference in people’s lives, every single day? We’re looking for an enthusiastic and forward-thinking Band 6 Occupational Therapist to join our dynamic Community Stroke Rehabilitation Team in Welwyn, Hertfordshire.

This is more than just a job – it’s an opportunity to play a vital role in someone’s recovery journey. Working closely with stroke survivors in their own homes and communities, you’ll be helping them regain independence, confidence, and purpose after life-changing events. You’ll bring your clinical skills, creativity, and compassion to deliver truly personalised, goal-driven therapy where it matters most.

You’ll be part of a supportive, multidisciplinary team that’s passionate about innovation, patient-centred care, and professional development. We believe in giving our therapists the autonomy to lead, the time to care, and the support to thrive.

If you love variety, value meaningful connections, and want to be part of a team that’s making a difference in stroke rehabilitation every day – we’d love to hear from you

Main duties of the job

Main duties of the job include the following:

  • Assess and treat stroke patients in their own homes or community settings, using evidence-based, goal-oriented interventions to promote independence and improve functional outcomes.

  • Work collaboratively as part of a multidisciplinary team to deliver integrated, person-centred care.

  • Develop and implement individualised treatment plans, focusing on areas such as self-care, cognitive function, upper limb rehabilitation, fatigue management, and return to work or meaningful activity.

  • Provide specialist clinical advice and support to patients, families, and carers to enable informed choices and effective involvement in the rehab process.

  • Supervise, mentor and support junior staff, assistants, and students to foster learning and maintain high standards of practice.

  • Contribute to service development, quality improvement initiatives, and audits to help shape and improve the delivery of the service.

  • Maintain accurate, timely clinical records and contribute to care planning, discharge planning, and onward referrals.

  • Promote self-management, empowering patients to build confidence and sustain progress beyond formal rehabilitation.
