HUC provides the NHS 111 and Out of Hours GP service to over 4.5 million patients across Hertfordshire, Cambridgeshire, Bedfordshire, West Essex and the South West of England. We are seeking highly skilled, motivated ACPs to join our Clinical Assessment Service (CAS), supporting patients 24/7 across our network.
Working remotely or from one of our contact centres, you will conduct clinical assessments via telephone and video, using cutting-edge digital platforms. Our service focuses on reducing unnecessary emergency department attendances and ambulance dispatches by offering safe, effective, and timely clinical management. You will be part of a dynamic multidisciplinary team including GPs, ACPs, ENPs, ECPs, Urgent Care Practitioners, and operational staff.

Main duties of the job

Autonomously assess, diagnose, treat, and discharge patients with a range of minor illnesses and injuries via telephone and video consultation.

Manage minor illness, minor injury, and minor ENT and ophthalmic presentations, offering appropriate care or onward referrals.

Prescribe medication electronically and refer patients to primary, secondary, community, or emergency services as required.

Deliver health education advice, promoting self-care and supporting patients to make informed decisions about their health.

Recognise acutely ill patients and initiate urgent interventions or escalate appropriately to ensure patient safety.

Identify safeguarding concerns and make referrals in line with safeguarding policies and best practice.

Maintain clear, accurate, and contemporaneous clinical records, contributing to audits, service improvements, and key performance targets.

Engage in professional development, working flexibly to meet service demands, attending meetings, and adhering to HUC’s governance and clinical standards.

Essential
Essential criteria
- NMC registered adult/dual trained Nurse or HCPC registered Paramedic
- University accredited minor illness qualification (Level 6/Level 7) or equivalent
- University accredited minor injury qualification (Level 6/Level 7) or equivalent
- University accredited advanced clinical assessment qualification (Level 6/Level 7) or equivalent
- MSc in Advanced Clinical Practice or equivalent credentialing
- Non-Medical Prescribing Qualification
- Triage/streaming experience
Desirable criteria
- Experience of working in an Emergency Department/Urgent Treatment Centre/GP Practice/Out of Hours Service
- Experience of conducting Telephone Consultations
- Experience managing End of Life care patients including prescribing/administering Just In Care medications (Including Controlled Drugs) where required/appropriate
- Experience with writing up inpatient prescription and medication administration records (Drug Charts)
- Evidence of audit or research experience
