Job overview

As an Urgent Care Practitioner, you will play a pivotal role in the clinical assessment and care of acutely unwell patients, delivering support across diverse settings including patients’ own homes and care/residential environments. Working as part of a multi-disciplinary team alongside therapists and social workers, you will assess individuals presenting with undifferentiated and undiagnosed conditions. Your responsibilities will include offering expert advice, making appropriate referrals, and providing input into social care interventions to help prevent hospital admissions. You will also contribute actively to maintaining high standards of care and ensuring our service aligns with both organisational and national quality objectives.

Main duties of the job

  • Deliver evidence-based interventions and treatments following clinical assessment.

  • Recognize and respond to signs of acute deterioration, arranging urgent or emergency referrals when necessary.

  • Work autonomously to manage complex and unpredictable caseloads, adapting interventions to individual patient needs.

  • Maintain accurate, timely clinical records in accordance with professional and regulatory standards.

  • Collaborate with General Practitioners and other healthcare professionals to ensure seamless patient care.

  • Support the development of clinical skills and knowledge by engaging in continuous professional development.

  • Recognize personal limitations and seek timely support or supervision from senior colleagues to ensure patient safety and care quality.

Qualifications/Experience

Essential criteria
  • NMC Registered Nurse/HCPC Registered Paramedic
  • Evidence of relevant further study including degree level studies or equivalent
  • Evidence of working with a variety of patients requiring triaging, assessment and treatment
  • Hold a current UK driving license
Desirable criteria
  • Systematic approaches course or equivalent
  • Minor illness/Minor injury qualification
  • Experience of utilising PGDs under specific conditions
  • Experience working in emergency department or fast moving assessment area
  • General Practice experience including out of hours setting
  • Working in a call centre environment providing healthcare advice
