Main duties of the job

  • To work as part of a community team which provides mental health services.
  • To focus exclusively on the needs of carers requiring support (including assessment of their needs) and act as an expert within geographical area of responsibility.
  • To support community team members in understanding best practice approaches to assessing the needs of and supporting carers.
  • To work within approaches outlined through the Care Act 2014 and local policy to ensure timely access to support for carers of people with a mental health issue.
  • To assist the senior social worker to ensure the integrity of the HPFT Carer Pathway within geographical area
  • To ensure that the wellbeing of carers is maintained at all times through promoting recovery oriented practice.

Detailed job description and main responsibilities

Duties and Responsibilities

To carry out direct work with carers including:

  • Carer assessments and reviews
  • Carer contingency plans and wellbeing plans
  • Providing information and support to promote and enable carer recovery
  • Holding a caseload of carers,
  • Supporting carers to develop coping skills and achieve their wellbeing goals
  • Accompanying carers to appointments / meetings and performing a range of practical tasks, relating to their identified needs
  • To support carers to understand the recovery goals, care plan and advanced directives and/or statements of the person they care for and to be involved in planning and implementing these
  • Support carers to understand their entitlement to a carer’s assessment and the support that is available to them from HPFT and other services
  • To facilitate access to community groups and networks that enable carers to participate in community activities, in order to maximise their opportunities to have a positive identity and socially valued roles
  • To support the multi-disciplinary team to develop their understanding of carer needs and services, by giving support, guidance and information including:

    • Developing , maintaining and sharing knowledge of local and national resources for carers
    • Working within, and promoting, the HPFT carer pathway and the Triangle of Care
    • supporting team members who also provide services to carers.
    • Having a focus on the right of carers at all times, including an understanding of the Care Act 2014 and Trust policies and strategies.
    • supporting care co-ordinators in liaising with local acute services to ensure support for carers is joined up across the care pathways
    • Helping to identify early signs of ‘carer breakdown’ by monitoring the wellbeing of carers and linking with local acute and crisis services in relation to this
    • Working with care co-ordinators and managers to respond to urgent situations emergencies or crises using local protocols, e.g. implementation of carer contingency plans
    • To liaise regularly with care co-ordinators and the multi-disciplinary team.

  • Contributing to the development of carer services and own practice by:
    • Participating in Trust-wide peer professionals practice meetings with other carer wellbeing and support workers
    • Contributing to the development of structured and semi-structured group work with carers within the community settings, and to contribute to the research of local need and the development of such groups to meet it.
    • Participate in team meetings and forums; and communicate with team members, carers and other agencies to provide an effective service to carers and to contribute to the development of carers services in HPFT
    • Using supervision, Trust and team policies, training etc. to develop skills and knowledge

  • To undertake such other duties as may be determined from time to time within the general scope of the post relating to support for carers, friends and families.

QUALIFICATIONS/EDUCATION/TRAINING

Essential criteria
  • NVQ level 3 qualification or mental health certificate level 3 or equivalent or agreement to undertake such training as soon as resources permit.
