Job overview
Are you ready to start a new career pathway working within a joint enterprise of Health and Social Care?
You could become part of this combined Service bringing the unique benefits of a wrap round recovery service from hospital discharge. The principle of the service ensuring that recovery involves working towards the person being as independent as possible through enablement.
This is a newly developed post working across Health and Social Care, supporting recovery and delivering enablement in all areas of daily life including personal care and supporting nutrition, whilst also monitoring the patient’s condition. Contributing to an overall plan of care, which includes the support of the multi-disciplinary teams from Health and Social Care.
Main duties of the job
You will play a pivotal role in delivering high-quality, person-centred care within the Home First service. You'll provide advanced assessment, complex case management, and operational support, while also guiding and developing colleagues to maintain excellence across the team.

Qualifications
Essential criteria
- Level 3 Diploma in Health & Social Care or equivalent
- Working Towards Level 5 qualifications
- Holding or willingness to work towards Qualified NVQ Assessor
- GCSE English & Maths (Grade C/4 or above) Bachelor’s degree (BA/BSc), NVQ Level 6, or equivalent
- Full UK driving licence
- Moving & Handling Train the Trainer qualification
Experience
Essential criteria
- Background in Adult Social Care and or Health Care or previous first line supervisor/line management experience.
- Experience of supervising staff.
- Experience of working to contract specifications and delivery to required standards.
Skills
Essential criteria
- Ability to undertake and document formal risk assessments
- Ability to accurately assess client needs
- Ability to communicate effectively with all levels of staff, People using the service and external contacts as required by the job
- Ability to prioritise own workload in line with operational procedures and standards

- Ability to work within national legislation and Corporate and Directorate policies and procedures relating to Health & Safety and those applicable to Health Appreciation of Diplomas in the management of care and relevant assessment techniques.
- Comprehensive awareness and understanding of the Outcomes as set out in the Health & Social Care Act 2008.
- Awareness of overall policies/procedures of department.
- Excellent understanding of enablement and how it fits the assessment process.
- Thorough awareness of legislative framework relating to the provision of enablement and domiciliary services.
- In depth knowledge of Safeguarding (Adult Protection) issues and need to promptly refer any concerns to Line Manager.
- Awareness of Data Protection and confidentiality issues.
- Awareness of and compliance with KCC equality and diversity policies, procedures and legislation.
