Prif ddyletswyddau'r swydd
You will promote excellence in clinical practice providing a visible presence and strong professional leadership across adult community nursing services.
A senior nursing role which draws on a variety of community service experience to provide expert clinical advice, guidance and support to our community nursing teams.
You will be an advocate for District nursing, to role model the professional standards, work alongside and support colleagues in the delivery of high-quality evidence-based care to our community nursing patients, cares and their families.
You will be instrumental in the development of clinical practice following clinical incidents, undertaking investigations, analysis and the identification of themes from patient safety incidents, after action reviews, and patient feedback to identify learning and ensuring the sharing of lessons learnt or outcomes with staff
and support the teams in the development of actions in clinical practice to enhance the care being provided by our community nursing teams.
Additional Working Pattern: 30.00 across Monday to Friday including on call element to role covering out of hours on a rotational basis.
