Job overview

An exciting and rare opportunity has arisen for Kent & Medway ICB for a senior project role leading the operational and strategic alignment and transformation of the All Age Continuing Care and Specialist Placement service into a single service delivering services equitably, efficiently and affordably across the ICB area.

The first stage of this project will require the completion of a paper to include an options appraisal reviewing all current contracts and services delivering AACC and Specialist Placements across Kent and Medway. Collating this information into an indepth analysis comparing the risks and benefits of the options for a single service provision to be delivered either in house or out sourced.

Main duties of the job

The expectation of this role will be to then lead the project of transformation consisting of 4 workstreams.

To align all aspects of the AACC and specialist placement service into a single equitable service covering the aspects of: Operational Policy and process; IT and Digital; Workforce and HR; and Communications.

With the aim of ensuring all aspects of service delivery, service development, quality, clinical effectiveness, value for money and patient safety is considered. Also supporting the AACC and Specialist Placement service leads to develop and implement a clear vision and strategic goals for continuous service development and improvement for the future.

The All Age Continuing Care service sits within NHS Kent and Medway Integrated Care Board (ICB) providing All Age Continuing Care and Specialist Placement services to the North Kent Localities. East and West Kent All Age Continuing Care services are currently delivered by South Central and West CSU.

This service includes delivering the co-ordination of functions of Continuing Health Care Assessment, Funded Nursing Care, Childrens Continuing Care, MH Sec 117 and Neuro Rehab
