Job overview

We are looking for a dynamic, forward-thinking consultant physician in Geriatric Medicine to provide clinical leadership for an integrated community approach to frailty in Partnership (HCP).  The role will be employed by Dartford & Gravesham NHS Trust (DGT), but jointly hosted by both the Trust and HCRG Care Group, who provide Community Services in DGS.  The post holder will collaborate with a range of local health and care system partners and services, including primary care, community services, secondary care consultants, mental health and dementia services and the local hospice

Main duties of the job

The role will provide expert clinical leadership, decision making, supervision, triage and care in the community, across primary and community based services, reaching in to and working in an integrated way with the acute frailty team.  This will include: -

  • Providing senior clinical leadership for the continuous quality improvement and design of evidence-based community frailty services, including: workforce development; integrated care pathways, and personalised care, fostering an inclusive and collaborative approach.
  • Provide professional and clinical leadership to the community hospital medical team.
  • Undertaking triage, Comprehensive Geriatric Assessment, diagnosis, treatment, care and care planning for complex frail patients, including onward referral where appropriate.
  • Working closely with Integrated Neighbourhood Teams for Frail and Complex patients

Detailed job description and main responsibilities

This will be mutually agreed between the appointee and the Clinical Directors at both DGT and HCRG Care Group after the appointee commences the post. This Job Plan represents the indicative initial weekly programme of work until it is agreed with or without changes between the appointee and the Directorate. The job plan will be tailored to meet the specialist interest and accreditation competencies of the appointee. The job plan will be signed by the Clinical Director and will be effective from the commencement date of the appointment. The Job Plan will then be reviewed at 3 months, then annually, following an Appraisal Meeting. The Job Plan will be a prospective agreement that sets out a consultant’s duties, responsibilities and objectives for the coming year. This will cover all aspects of a consultant’s professional practice including clinical work, teaching, research, education and managerial responsibilities, and provide a clear schedule of commitments, both internal and external. In addition, it should include personal objectives, and details of the support required by the consultant to fulfil the job plan and the objectives.

Qualifications

Essential criteria
  • • Full GMC Registration, MRCP
  • • MBBS or equivalent
  • Holds CCT or equivalent or will obtain within 6 months of interview date
  • Applicants that are UK trained, must also be a holder of a Certificate of Completion of Training (CCT)

Clinical Skills

Essential criteria
  • Wide experience in all aspects of Geriatrics and GIM
  • Experience of Multidisciplinary Team working at senior level.
  • Ability to take full and independent responsibility for clinical care of patients at consultant level
Desirable criteria
  • Demonstrates ability in managing common acute and chronic conditions, including emergencies
  • Experience of providing leadership

Management and Leadership Skills:

Essential criteria
  • Experience of service innovation and development Demonstrates an understanding of NHS Management and resources
  • Evidence of effective multidisciplinary team working and leadership supported by multi-source feedback or other workplace-based assessments
  • Evidence of effective leadership in and outside medicine Ability to supervise medical staff
