Prescribing Support Project Dietitian

Job overview

The aim of this role is to develop and standardise nutrition care pathways for adult patients requiring nutritional support management in primary and secondary care. You will take a key role communicating with a range of healthcare professionals across the system. You will be an expert in providing specialist advice training to healthcare professionals, educating and supporting patients, carers and other health professionals within the multidisciplinary team.

 

You will be responsible for optimising prescribing of adult oral nutritional supplements across Dartford, Gravesham and Swanley. As well as working collaboratively with Primary Care Networks, acute and community dietetic teams and community care homes to ensure patients have their optimal nutritional care along their treatment pathways

 

Main duties of the job

Lead and implement the nutrition advice and guidance to care homes initiative.

Lead and design a pathway for review of oral nutritional supplements (ONS) prescription requests on the electronic discharge notification (EDN) from secondary care to primary care.

Use local prescribing systems and GP noted to identify patients being prescribed ONS, review their nutritional care plan and modify these appropriately in conjunction with the GP/Practice pharmacist advising on product choices in cases of complex nutrition need.

Make recommendations to clinicians in General Practice and Community Hospitals on food-based recommendations and/or prescription of nutritional supplements for patients requiring nutritional support.

Directly advise on prescriptions and modify nutritional supplement recommendations based on nutritional status and to inform the GPs and community pharmacists. This will include recommending that a prescription is stopped (de-prescribing).

Person specification

• Degree in Nutrition -graduate Diploma or equivalent qualification

  • HCPC registered
  • Evidence of continuing professional development
Desirable criteria
  • BDA Membership
  • member of the BDA prescribing group

Knowledge and skills

Essential criteria
  • highly developed computer skills
  • able to teach effectively
  • familiar with EMIS or similar
  • knowledge and experience of working with different cultures
  • excellent communication skills both written and verbal
  • 4 years post grad experience in all aspects of dietetics
  • excellent knowledge in audit
  • lead projects
