Community Cardiac Rehabilitation Nurse

NHS AfC: Band 6

Trosolwg o'r swydd

The NHS is changing. More emphasis on community-based care means there’s never been a better time to join us.

We believe in treating the whole person, not just the condition. We're looking for a Community Cardiac Rehabilitation Nurse to join our Folkestone based team.

This established, award-winning Cardiac Rehabilitation Service is a multi-disciplinary team, comprising nurses, physiotherapists, exercise physiologists, counsellors, cardiac rehab assistants, practitioners’ administrators and volunteers.

Services are provided in a variety of locations, including in a patient’s home, outpatient clinics in Health Centres and GP practices and community venues such as Leisure Centres and village halls, within the Ashford, Canterbury, South Kent Coast and Thanet localities.

We offer physical, social and psychological rehabilitation through a wide range of options, tailored to meet individual patient needs and preferences.

For the right applicant we are happy to offer this as a Band 5 developmental post. We will work with you, offering structured support and development to give you the skills and training required to move you up to a Band6 post in an allocated time frame.
