Detailed job description and main responsibilities

In this role, you will play a vital part in providing holistic, patient-centered care. You'll conduct comprehensive assessments, considering all aspects of a patient's well-being - mental, physical, social, cultural, and more. By working collaboratively with patients, their families, and other healthcare professionals, you'll develop personalized care plans that address their unique needs.

As a clinical leader, you'll champion best practices and quality improvement initiatives. This is a chance to make a significant impact on the care provided within our setting.

Experience

Essential criteria

Registered on NMC
One year experience at Band 5 or above in hospital or community health care
Evidence of continued professional development, appropriate to role
Clear understanding of CPA and care coordination role

Desirable criteria

Post-registration certificate ENB 998. Possession of specialist nursing or other relevant highter academic or skills focussed awards
Post registration mentorship qualification

Skills/Knowledge

Essential criteria

Good IT skills
Good understanding of the principles of the Recovery Approach

Previous experience in the field of work

Probation Period. New employees external to Kent and Medway NHS and Social Care Partnership Trust are subject to a six-month probationary period.
