  • Are you a registered nurse already working with people with diabetes?
  • Do you have excellent interpersonal skills and sound clinical assessment skills?
  • Do you have the confidence to work alone, putting the patient at the heart of what you do; and to get involved within a multi-disciplinary team?
  • Are you passionate about supporting patients holistically and working as part of team to support those with diabetes?

Prif ddyletswyddau'r swydd

  • Provide specialist, evidence-based nursing care to support and facilitate high-quality, individualised care for people living with diabetes and their families/carers.
  • Manage a community diabetes caseload (out-patients and housebound), applying advanced diabetes knowledge and skills to support individuals with complex needs.
  • Demonstrate critical application of in-depth knowledge in diabetes care, with associated management and leadership responsibilities, including completion (or commitment to undertake, if developmental) a postgraduate module in diabetes.
  • Work collaboratively with community services, general practice, care homes, nursing homes, and rehabilitation hospitals to reduce hospital admissions, improve diabetes outcomes, and enhance quality of life.
  • Support, encourage, and mentor junior members of the diabetes team in managing caseloads, ensuring regular clinical supervision and contributing to management responsibilities.
  • Ensure the care delivered by the diabetes team is culturally sensitive, respects individual health beliefs, and aligns with the principles of ‘Language Matters’.
  • Update, organise, and deliver structured education programmes for people living with Type 1 diabetes in group settings.

Qualifications, training, and knowledge

  • Recognised degree/diploma in Nursing.
  • Registered Nurse on Part 1 of the Nursing & Midwifery Council register.
  • Hold a valid UK driving license and be willing to drive between sites and undertake home visits.
  • Post-graduate module in diabetes (or willingness to undertake if developmental).
  • Evidence of continuous professional development
  • Non-Medical Prescriber or working towards/willingness to undertake.

 

  • MSc in diabetes.
  • Diabetes specific training and qualifications.
  • Experience in delivering structured education.
  • Teaching qualification.

Experience

  • Relevant experience as a Band 6 for at least 2 years.
  • Evidence of managing junior staff, including appraisals/supporting staff development.
  • Be able to work and make decisions autonomously.
  • Ability to work collaboratively with other healthcare professionals and agencies to ensure the needs of the patients are met.
  • Experience of clinical assessment, letter writing and electronic systems.

 

  • Experience working in a community setting.
  • Experience in providing specialist education to other healthcare professionals.
  • Experience in line management

 

Skills and Abilities

  • Good level of IT ability, including all Microsoft programmes.
  • Good verbal and written communication skills.
  • Maintain accurate and up-to-date clinical records using and electronic database.
  • Ability to prioritise and organise workload effectively.
  • Flexible and responsive to the needs of the service.
  • Ability to recognise own limitations and to refer appropriately, when necessary, in accordance with professional code of conduct.
  • Knowledge of clinical governance and risk management.
  • Ability to develop effective care plans.

 

  • Experience using RIO electronic patient record system.
  • Experience of delivering PowerPoint presentations.

Specialist knowledge/

expertise

  • Analyse continuous glucose monitoring data and provide written reports.
  • Assess, plan, implement and evaluate diabetes care management (face-to-face and remotely).
  • Ability to triage, prioritise and coordinate referrals to the diabetes caseloads.
  • Working knowledge of national guidance on diabetes.
  • Knowledge of international guidance on diabetes (EASD/ADA).
  • Knowledge of current issues and trends in relation to long-term conditions.
  • Evidence of keeping up to date with new treatments, techniques and developments in diabetes.
  • Supportive of clinical audits and quality improvements to improve the delivery of diabetes care.
  • Able to liaise with appropriate healthcare professionals to facilitate a timely hospital discharge.
  • Knowledge on relevant articles published in professional journals.
  • Experience in participating in research and/or audits.

Planning and organisational skills

  • Experience within the area of diabetes.
  • Able to contribute to the development of the team and service provisions.
  • Able to comply with national and organisational polices and procedures.
  • Ability to manage own diary, allocations, clinics and home visits efficiently.
  • Effective delegation and organisation of junior members of staff, and students.
