Job overview

  • To work as a key member of the transfer/discharge process within Medway
  • Ensure effective communication and developing sound relationships with colleagues, health and social professionals from within and external to Medway Community Healthcare, as well as colleagues in the voluntary and private sector

Main duties of the job

The post holder will be a key member of the senior nursing team, assisting with the maintenance of structures, systems and processes in place so that admission avoidance and discharge activity is effectively coordinated across Medway NHS Foundation Trust within an integrated approach.

The post will involve liaison with other intermediate care services within the Medway area, primarily the community rehab teams but also our Council colleagues

 Work with multi-disciplinary team to facilitate timely and appropriate clinical management of patients

 Develop and maintain good working relationships with inpatient and community services within the organisation and with external bodies

Attendance and active participation at daily Board Rounds, fully contributing towards the safe and timely discharge of individual patients.
