We are seeking an experienced Diabetes Nurse to join our Community Nursing Team. This autonomous role involves leading and managing the diabetes care of patients on the community nursing caseload.

You will work independently within the community nursing team, taking responsibility for assessment, treatment planning, and ongoing management of diabetes, including oral therapies and insulin. A strong understanding of diabetes medications and insulin titration is essential. Independent prescribing is desirable.

If not a prescriber, you must be confident liaising with GPs to suggest medication changes and insulin adjustments until your non medical prescribing course is completed. This role is ideal for a confident, proactive clinician with specialist diabetes knowledge and a passion for high-quality community care.

Qualifications

  • Evidence of extensive experience in Diabetes care and/or a Diabetes qualification.
  • Registered health care professional (NMC registration) / significant relevant experience in health improvement environment.
  • Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing.
  • Evidence of Post Graduate Specialist qualifications relevant to area of practice (examples: SPDN, Mentorship, Coaching, Clinical Expertise Pathway)
  • Full UK/EU driving licence

  • Clinical Examination Course
  • Non-medical Prescribing qualification.
  • Management or supervisory qualification/ programme of study

Experience

  • Experience with the use of Microsoft Office/ eRoster/ Appraisals/ Training Matrix/
  • Relevant experience in area of practice
  • Experience of leading and managing teams.
  • Experience of supporting the development of protocols and service development plans with practical experience of change management.
  • Practical experience in managing health/ team related projects.
  • Experience of managing complex patients and caseloads.
  • Experience of supporting and being a role model to others (mentor / educator / supervisor) through clinical supervision/ 1:2:1 and personal development plans.

Knowledge

  • Understanding of National Service Frameworks and NICE guidance relevant to care setting and its application across health care services.

  • Proven ability to develop effective partnerships to achieve practical outcomes across organisational boundaries including statutory organisations, external agencies, community and voluntary organisations
