Job overview
Community Nursing Teams provide interventions for housebound
patients aged 18 and over delivering personalised nursing care to
those with complex and long-term conditions. The aim of our service is to enable patients to stay safe and well in their chosen environment.
The service actively works to prevent avoidable admissions by providing tailored care that meets the patient need and assists them in actively achieving their long-term health goals and self-care where possible. The service operates in teams aligned to Primary Care Networks and provides integrated care founded in the framework of the Multidisciplinary Team.
Main duties of the job
Your daily activities will be varied and will include some of the following:
- Palliative and end of life care provision
including symptom control, advance care
planning and verification of death - Administration of medication including IV
therapy - Wound management including leg ulcers,
post-operative surgical wounds, drains - Bladder and bowel care
- Health promotion, education and support to
manage long-term conditions

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board (ICB). Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

Person specification
Qualifications, training and experience
Essential criteria
- You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing.
- You will be a registered health care professional with the Nursing and Midwifery Council
- You’ll need to have recent experience of healthcare gained through education, work experience or previous employment. This role is ideal for a newly qualified graduate nurse.
- Full UK Driving License with a car
Knowledge & skills
Essential criteria
- You’ll need have sound communication skills in order to understand and communicate both verbally and in writing.
- You will need to be able to be able to use IT systems and databases.
Desirable criteria
- You will fully understand the role and remit of a health care worker.
- You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks.
- Having an awareness of developments within the NHS would be an advantage.
Working environment
Essential criteria
- A full, valid UK driving licence and the ability to travel independently are essential for this role. Should you require workplace adjustments under the Equality Act 2010, we are committed to supporting you to meet the mobility requirements of the role wherever possible.
