Job overview
We are seeking a consultant frailty-focussed geriatrician who is keen to support community expansion as the 10 year NHS plan embraces initiatives to shift care from acute to community. This post requires a flexible Consultant Geriatrician who can, where resources permit, work collaboratively across organisations, including potential involvement with MTW Hospital. Our seven‑day service offers rapid assessment, near‑patient testing and hospital‑at‑home interventions, including intravenous therapies developed during the Covid pandemic. You will lead on Comprehensive Geriatric Assessment (CGA), anticipatory care and advance care planning through domiciliary visits, ensuring holistic, person‑centred support.
The role also includes training and supporting care home and primary care staff to deliver holistic assessments and personalised anticipatory care plans that reduce avoidable deterioration. During the Covid pandemic, our team provided vital on‑site assessments, virtual ward rounds and delivery of the Active Covid Bundle, shaping our innovative approach to acute care at home.

Main duties of the job
This role is for a Consultant Geriatrician within the Community Home Treatment Service at KCHFT, working closely with Primary Care, Secondary Care, Social Care, Mental Health, and Commissioners to expand support for frail patients in the community. The service currently includes geriatric consultants, specialist and specialty doctors, a consultant practitioner, GPs, advanced clinical practitioners (ACPs), and trainee ACPs, with plans to grow the team further.
The West Kent Frailty Home Treatment Service is an innovative multi-disciplinary team delivering hospital-level care to people at home, offering CGA, IV treatments, bedside diagnostics, medication reviews, and oxygen or nebuliser therapies when needed. It’s a fantastic chance to develop clinical skills while supporting those living with frailty, especially in the evolving context of virtual ward models, and to work alongside strong networks in primary care, acute services, and paramedic trusts, delivering patient-focused care with a blend of creativity, clinical expertise, and great communication.

Qualifications
Essential criteria
- On the GMC specialist register or within 6 months of CCT.
- Professional qualifications (MRCP, MRCPCH by examination or equivalent).
- CCT accreditation within area of speciality for example in Paediatrics or within 6 months of acquiring the certificate.
- Full GMC registration.
Desirable criteria
- Any further postgraduate qualifications relevant to speciality
Experience
Essential criteria
- Significant Higher Specialist Training within an area of speciality

Essential criteria
- Proven ability to develop effective partnerships to achieve practical outcomes across organisational boundaries including statutory organisations, external agencies, community and voluntary organisations
