Job overview

We’re seeking a talented Nurse to join our Pathway 3 Service in the Maidstone and Tunbridge Wells area. 

Pathway 3 focuses on supporting individuals discharged to local care homes who need further assessment to decide their long-term care plan after leaving the hospital. 

Our team collaborates closely with the acute trust Discharge Liaison Teams, Kent County Council Social Services, All Age Continuing Health Care, and local community services, including voluntary organizations.

Main duties of the job

This role involves visiting patients in their homes to assess their needs and help develop long-term care plans. 

As it is community-based, having a valid driving license and access to a car is essential. 

Strong communication and compassion are key, as you’ll work closely with partner organizations, patients, relatives, and carers to ensure everyone involved in the individual’s care and assessment understands and supports the process.

Qualifications

Essential criteria
  • Full NMC registration
  • Evidence of Specialist qualifications relevant to area of practice
  • Completion of mentorship
  • Full UK driving licence, with access to vehicle
Desirable criteria
  • Evidence of continued professional development
  • Completion, or working towards Masters level qualification

Experience

Essential criteria
  • Experience of working with patients who have long term health conditions, dementia
  • End of Life support

Skills

Essential criteria
  • Competent using all IT systems including MS Word and Excel
Desirable criteria
  • Experience of using RIO and Sunrise
