MCH are looking for a self-motivated, committed and dynamic Stroke Rehabilitation Assistant to join our friendly stroke team.
This exciting opportunity will allow you to work with stroke survivors both in an inpatient setting and in their own homes. You will work alongside all disciplines of the Multi Disciplinary Team and will carry out ongoing therapy plans as directed by the qualified members of the team.
The service covers diverse communities in Medway and Swale and gives you the chance to work with your patient from hospital discharge to home.
This post is part time 16.5 hours and is based predominantly at Sheppey Community Hospital.
The stroke pathway consists of inpatient Stroke Rehabilitation Units (SRU) at Harmony House and Sheppey Community Hospital, as well as Early Supported Discharge Team (ESD) and Stroke Community Assessment Rehabilitation Team (SCART) for longer term rehab. You will have the opportunity to support your patients to achieve their goals for as long as clinically indicated.
Main duties of the job
Main duties:
- To assist the qualified members of the team in delivering therapy to patients who have had a stroke.
- To deliver individual care plans designed by qualified members of the stroke team under supervision.
- To participate in and support qualified team members in stroke prevention
- To work alongside colleagues from within Medway Community Healthcare and other organisations to carry out enabling care tasks on a daily basis.

Detailed job description and main responsibilities
To work as a member of the stroke interdisciplinary team, providing rehabilitation to stroke patients in hospital and community settings, under the guidance of professionally qualified members of the team and in liaison with the manager of Stroke Services and other members of the interdisciplinary team:
- Assist in the treatment of patients following a stroke in Medway and Swale
- Implement a treatment and rehabilitation program for the patient as instructed by the qualified team members.
- Assist in the on-going development and evaluation of the stroke service
