We’re looking for dynamic Occupational Therapists to join our Living Better, Living Well Service (formerly Post Covid).
 
This is an exciting opportunity to shape and develop a growing service that’s making a real difference to people’s lives. We are in an exciting period of service expansion into the boroughs of Bexley and Greenwich. The successful candidates will have the opportunity to develop/refine their service development skills. 
You’ll work as a key member of a supportive and forward-thinking multi-disciplinary team, using your specialist skills to deliver holistic, person-centred care and be involved in service development and quality improvement.

The Occupational Therapist in the Living Better, Living Well Service is responsible for the day-to-day professional management of all aspects of Occupational Therapy intervention to people accessing this service either in their own homes, at clinics, in the community or by virtual means across the boroughs of Bromley, Bexley and Greenwich.

The post holder is expected to provide a comprehensive Occupational Therapy service to patients referred with complex multiple pathologies who exhibit a range of physical, psychological, neurological, emotional, behavioural and social problems.

The post-holder is expected to contribute to the resource base and implement evidence based practises across the pathway.

To provide an Occupational Therapy service of the highest possible standard for those referred to the service by: 

Clinical:

  • Assessing patients through virtual or face to face means, independently making autonomous decisions in the clinical assessment of patients with complex health and social care needs. 
  • Agreeing goal orientated programmes in conjunction with the client, his/her family and other professionals involved and planning and implementing appropriate intervention. 

  • Devising rehabilitation programmes using the most up to date available evidence and guidance to enable clients to develop skills that will improve their performance and independence as well as health and wellbeing. 

  • Liaising with other health care professionals, making referrals, making recommendations where necessary to reduce the effect of disability on health and wellbeing. 

  • Participate in health promotion/education and give advice as necessary to clients and their carers. 

  • To formulate and evaluate individualised management and / or treatment plans, using clinical reasoning and utilising a wide range of treatment skills and options in order to formulate a specialised programme of care e.g. supported Self-management. 

  • Monitor client’s health and medical status; reporting any changes or deterioration to the MDT involved in the client’s care accordingly.

  •  To attend MDT meetings to present patients when appropriate and to discuss recommendations identified at the initial assessment.

  • To carry a caseload of patients as an autonomous practitioner, including those with highly complex presentations.

  • To delegate clinical work to the Therapy Assistant, as appropriate in order to meet needs of the patient group.
