Main duties of the job
This is a great opportunity for those who feel they no longer have the time to give basic holistic care to their patients.
The team work to support and empower each other to deliver the highest standard of person-centred care.
The service covers from 7am to 7pm, 7 days a week. You will need to work a mixture of the following shift times to cover the needs the service: 7am to 3pm, 8.30am to 4.30pm and 11pm to 7pm.
You will work as a member of a neighbourhood nursing team that delivers person centred integrated care in the community. The aim of the service is to support people to remain as independent at home as long as possible, proactively facilitating self-care.
Care is focussed on improving self-reliance and empowerment; seeing patients as a whole and supporting as required their health, social and psychological needs.
We are seeking people who are adaptable and collaborative, with the potential to grow, and the ability to plan, prioritise and organise their work. We ask that our senior people inspire, motivate, and unleash potential in others!
