To work as a member of the stroke interdisciplinary team, providing rehabilitation to stroke patients in hospital and community settings, under the guidance of professionally qualified members of the team and in liaison with the manager of Stroke Services and other members of the interdisciplinary team:

  • Assist in the treatment of patients following a stroke in Medway and Swale
  • Implement a treatment and rehabilitation program for the patient as instructed by the qualified team members.
  • Assist in the on-going development and evaluation of the stroke service

  • To assist the qualified members of the team in delivering therapy to patients who have had a stroke.
  • To deliver individual care plans designed by qualified members of the stroke team under supervision.
  • To participate in and support qualified team members in stroke prevention
  • To work alongside colleagues from within Medway Community Healthcare and other organisations to carry out enabling care tasks on a daily basis.

1.    Communication and relationship skills

•    Exercise good clinical practice and assist the senior staff to achieve high morale in the team.
•    Communicate any issues affecting the service to the team lead
•    Participate in service meetings.
•    To work in a team structure, liaising with and maintaining an adequate/effective communication with colleagues and other members of the multi-disciplinary team to the benefit of patients and staff.
•    The post holder is expected to communicate effectively to the identified member of the multi-professional team, provide professional, up to date and accurate information regarding patient's progress.
•    To act in a professional manner at all times.
•    Be part of and to work towards enhancing the MDT's.

2.    Knowledge, training and experience

•    Good general education (to GCSE or equivalent)
•    NVQ level 2 or equivalent with evidence of working towards NVQ level 3.
•    Ability to attend required training courses/sessions which are required by the remit of the post.
•    Commitment to continued education and personal development.
•    To understand the philosophy of rehabilitation.
•    Good written and verbal communication skills.
•    IT skills.
•    Able to demonstrate empathy and be able to motivate and persuade patients to take part in rehabilitation programmes.
•    Ability to work as part of a team.
•    Reliable, flexible and organised.
•    To have knowledge and awareness of infection control policies and procedures.
•    To have a knowledge and understanding of the application of skin care/wound care and how to apply it into practice

3.    Analytical and judgement skills

•    Ability to recognise when to call for assistance and/or report changes in the patient.
•    To handle written and verbal enquiries in a professional manner and to deal with them promptly and effectively

4.    Planning and organisational skills

•    Decision making and acceptance of personal accountability for one's own actions.

5.    Physical skills

•    To assist clinicians in the rehabilitation of patients using a range of therapeutic techniques.
•    To organise and run group activities as guided.
•    To assist/supervise clinicians with the moving and handling of high risk patients in preparation for rehabilitation or during therapy sessions according to the patient's risk assessment.
•    To do some minimal clerical duties.

6.    Responsibility for patient / client care

•    Ensure the patient's needs are met as identified in the care plan.
•    To undertake individual rehabilitation programmes with the patient.
•    To assess patient's progress and within the parameters set by the clinicians, make alterations to the existing treatment plans and feedback progress.
•    To treat patients individually or in a group without direct supervision.
•    To support patients within home assessments.
•    Using empathy, sensitivity and good interpersonal skills to encourage patients and their carers in an active approach to regaining and maintaining personal independence.
•    To maintain patient's privacy and dignity at all times.
•    To use effective communication tools, verbal and non-verbal to communicate treatment programmes to patients who may have barriers to understanding or communication e.g. blind, deaf, dysphasic or who may not have English as their first language

7.    Responsibility for policy and service development implementation

•    Identify any problems, which may interfere with the smooth running of the service and together with the senior staff , discuss ways of resolving these. Put forward suggestions and ideas to the team lead and encourage others to do the same.
•    Complies with organisation operational policies and procedures.
•    To be proactively aware of health and safety aspects of your work (area) and implement any policies, which may be required to improve the safety of your work (area), including your prompt recording and reporting of (near miss) accidents or concerns.

8.    Responsibilities for financial and physical resources

•    To be responsible for the ordering and the issuing of  aids and equipment  and for checking and maintaining these aids following infection control and health and safety protocols inclusive of normal hygiene cleansing.
•    To ensure that all resources are used appropriately to provide quality care.

9.    Responsibilities for human resources (HR)

•    Take part in orientation and induction programmes for new staff.
•    Participate in team training.
•    To participate in the appraisal process so that personal and organisational goals are met.
•    The post holder is expected to act in ways that support equality and value diversity, in accordance with legislation and national local policies.
•    By undertaking mandatory and professional study days/courses to ensure up to date evidence based practice.
•    Undertake individual performance review as required.
•    To maintain NVQ level knowledge and competencies by reading clinical/professional literature and research, study days/courses associated with.

10.    Responsibilities for information resources

•    Expected to ensure correct use of confidential information and sensitive information in line with caldicott guidelines.
•    Ensure that you record all interactions in relation to patient care and rehabilitation and appropriately/accurately and timely.
•    To be responsible for maintaining accurate and prompt records of all own clinical interventions (SOAP notes and communication to the MDT) in line with legal requirements and local and national standards.
