We are seeking someone who is adaptable and collaborative, with the potential to grow, and the ability to plan, prioritise and organise their work, to join our neighbourhood nursing service.

Due to the nature of the role, you will need a full UK driving license and access to a vehicle for this role 

 

Prif ddyletswyddau'r swydd

The neighbourhood nursing teams provide nursing care for patients, aged 18 and over who are housebound within the Medway area, delivering a range of personalised care and treatment to those with complex and long-term conditions, supporting people to stay safe and well at home. 

You will be a registered nurse or paramedic responsible for patients allocated to them and will assess, manage, plan, and deliver care, including being professionally and legally accountable.

You will lead and manage a Neighbourhood Nursing Team in Strood, geographically aligned to a locality and be responsible for the management of a caseload ensuring the provision of a high standard of nursing care to patients and their families in the community.  You will work as part of the integrated health care team, working within and across professional and organisational boundaries.  liaising with GPs, other professionals and statutory and voluntary agencies, they will deliver individualised care to patients. 

The service operates a 7am to 7pm service, 365 days in the year. You will need to work a mixture of the following shift times to cover the needs the service: 7am to 3pm, 8.30am to 4.30pm and 11pm to 7pm.

You will need to have a District Nursing qualification / community practitioner pathway or be willing to commence this training within 1 year of starting.

The small print

We are a small social enterprise providing NHS services, and as such, we cannot take overseas nursing candidates who do not already have full UK registration with the Nursing and Midwifery Council. We cannot support candidates with their OSCE preparation, OSCE exam, accommodation and pastoral care. As such, if you are not able to provide us with your NMC PIN on your application, we will not be able to shortlist you.

·     Recognised degree in adult nursing

·     Registered Nurse on Part 1 of the Nursing Midwifery Council register

·     Evidence of further study

·     Experience and competence in high-level clinical assessment of complex patients

·     Evidence of specialist knowledge in minor illness management, community nursing and advanced practitioner

·     Experience of using technology to facilitate provision of patient care

·     Experience of motivational interviewing

·     Knowledge of current issues and trends within community healthcare, urgent Care, long term conditions, intermediate care and the NHS in general
