Community Staff Nurse - Triage and Virtual Assessment.
Band 5 - 37.5 Hours per week
The Tameside & Glossop District Nursing Service are currently developing our district nurse liaison team and are looking to recruit a band 5 Triage nurses. Our service works across the 5 neighbourhoods of Tameside.
The role may require you to work across the localities as a service need requirement.

Main duties of the job
The post holder will work within the District Nursing triage team and wider a Multi-Disciplinary Team in the Community. The post holder will support the discharge plans for patients ensuring seamless transition to the District Nursing Service. They will be responsible to ensure good clinical practices and high standards of care are maintained in order to meet patient needs
supported by Community Specialist Practitioner in District Nursing (CSP). They will support the workforce to deliver evidenced based wound care, excellent End of Life care provision and evaluation of programmes of care for a specific group of Patient groups within the community.
