Job overview

An exciting opportunity has arisen for an experienced Registered Nurse to join the Integrated Diabetes Service. The focus of this role will be to work closely with our multidisciplinary team (MDT) to support adult inpatients across ELHT.

Our Integrated diabetes team is an early adopter of technology and uses examples of all current diabetes technology available to co-ordinate patient care, whether this is flash glucose monitors, insulin pumps and CGM.

The ELHT integrated diabetes service provides complex care to inpatients with diabetes across ELHT and BwD, the inpatient service is 7 days.

Main duties of the job

The post holder will jointly manage a caseload of inpatient’s whilst working alongside a highly skilled team of Adult Consultants, Adult Diabetes Specialist Nurses, Specialist Podiatrist and Specialist Dietitians. ELHT have been successful in receiving NHS England bid monies to develop our inpatient service. The role is to prevent unnecessary admissions, support inpatient care and timely discharge, and delivery training to HCP across the health economy.

Detailed job description and main responsibilities

 Working across sites within ELHT NHS trust, you will be expected to deliver patients centred advice, support and care for patients with diabetes within the integrated diabetes service, their families and carers. In line with National Service Framework (NSF) and the Nationals Institute for Health and Care Excellence (NICE) guidelines you will be expected to provide advanced assessment of patients’ needs and advise on appropriate diabetes management. Additionally, you will be expected to support patients through all stages of their diabetes care.

Your role will involve empowering patients with the skills and knowledge necessary to attain independence in self-management. Working as a Diabetic specialist nurse ward lead you will be responsible for maintaining and updating your own knowledge and skills, together with providing evidence in support of this. This is a 7-day service.

  • Access to transport for work purposes

Qualifications

Essential criteria
  • Degree and evidence of study at Master’s level (willing to undertake) or equivalent
  • Degree module in Diabetes management or equivalent experience
  • Registered Nurse Level 1
  • Non-Medical Prescribing Qualification (willing to undertake) or equivalent
  • DAFNE/ DAP Education (Willing to undertake) or equivalent
