Job overview
Band 3 Health Care Assistant, working with IV Therapy Team.
Successful candidate would be based at Breightmet Health Centre ( subject to change), clinics are based at Breightmet Health Centre, however, domiciliary visits are across the borough of Bolton, therefore successful candidate must have access to their own transport.
Main duties of the job
Venepuncture - in clinic and domiciliary visits
Clinic - Assisting with clinic, general duties within clinic setting, running bloods and weights clinic
Admin - assisting with admin where required
Stock - ordering stock and maintaining stock levels
Collecting medication from hospital, either taking to patients homes or returning to Breightmet Health Centre

You will work in partnership with patients, carers and other health and social care agencies to promote independence and develop individual plans of care relevant to patient need within their limitations to practice

Detailed job description and main responsibilities
Assist registered staff in clinic – bring patients to and from waiting are perform and accurately record patient observations, provide patient with refreshments, cleaning down of equipment between patient
Performing venepuncture either within clinic setting and domiciliary visits
· Communicate effectively with all patients and their relatives
Communicate with team members and feeding back to coordinator to ensure coordinated care, this may also include the wider MDT ( eg, haematology, gastroenterology, AAT)
Stock management – maintaining adequate stock levels, ordering stock via NHS supplies and Integra ordering systems.
· Assist with admin where necessary, in particular, during annual leave of team admin (LE2, CIVICA, preparation of notes, preparation for clinic)
· To provide support to the team leader, team coordinator, and other members of the team as appropriate and necessary. This will include supporting the flow of discharge from hospital to the IV Team, this may include printing of notes in absence of admin, stock equipment for new patient visit
Collection of medication from pharmacy, either to be brought back to base or delivered at patients home
· Documentation – effectively documenting patients notes, and updating of clinic records
To support the daily running’s of the team but to work only within scope of practice at all times
To provide support Admission Avoidance team where necessary through collaborative working

- Ability to travel across the locality independently in accordance with the Equality Act 2010
Education / Qualificiation
Essential criteria
- NVQ level 3
- Evidence of learning in a care environment
- Evidence of extended skills and completed competencies in venepuncture and clinical observations.
Desirable criteria
- Completion of care certificate
- Relevant qualifications or training to a higher level
Experience
Essential criteria
- Experience of working in teams under appropriate supervision as part of a multi-disciplinary team
- Insight into how to evaluate own strengths and development needs, seeking advice where appropriate
- Experience in Health and Social Care community
Desirable criteria
- Experience of working in community
