Job overview
This is an exciting opportunity for an experienced dietician to join the newly formed Proactive Care Team as part of Bolton's response to the NHS 10 year plan to move care from hospital to home.
The service operates Monday - Friday and offers flexi-time working arrangements, recognising the importance of balancing the needs of work with family commitments. The post is a 18.5 hours/week post.

Main duties of the job
To work as an autonomous practitioner within the Proactive Care Team, supported by a multi-disciplinary team, including primary care.
To perform therapeutic assessment of patients in their home, with diverse presentations and complex physical and psychological conditions, to provide a diagnosis and develop and deliver individualised treatment programmes.
To manage a defined caseload, using evidence based, client centred principles to assess, plan, implement and evaluate interventions.
To provide clinical education, advice and support for junior therapy staff and support staff.
To provide highly specialist knowledge, skills and advice to clients, and be a source of expertise for therapy staff, other members of the MDT and other agencies.
To supervise, educate and assess the performance of students.
To contribute to service research and audit activities.
To propose any service changes within own professional speciality area.

Qualifications
Essential criteria
- Degree in Nutrition & Dietetics or other relevant degree with PG Diploma in Nutrition & Dietetics.
- Registered with Health and Care Professions Council.
- Member of British Dietetic Association
Desirable criteria
- Member of North West & North Wales branch of the BDA
Experience
Essential criteria
- At least one year practising as a dietitian
Desirable criteria
- Experience of working with complex conditions, including frailty
Skills
Essential criteria
- Good communication skills, both written and verbal, with individuals and groups.
- Ability to give formal and informal presentations, and to plan deliver and evaluate teaching sessions
- Uses patient-centred approach when working with clients, and making appropriate use of motivational interviewing and dietary counselling skills.
- Able to work effectively and develop good interpersonal relationships with a range of people, and commitment to multidisciplinary team working.
- Basic IT skills: email, internet, Word, PowerPoint.
- Good time management and organisation skills, able to work to deadlines.
- Able to travel around Bolton. The post requires you to have access to a suitable vehicle to be used within your role subject to the provision of the DDA.

Knowledge
Essential criteria
- Evidence based knowledge of clinical dietetics
- Knowledge and understanding of Clinical Governance issues, professional code of conduct and confidentiality.
- Knowledge of NHS priorities, targets, NSF’s and NICE guidance
- Ability to conduct literature searches and critically evaluate clinical evidence.
Desirable criteria
- Knowledge around complex long term conditions, including frailty
