Job overview

Are you looking for a new wide ranging pharmacy experience? The ICSD Pharmacy Team plays a significant role in supporting patients following an acute hospital discharge or to prevent avoidable admissions to hospital during periods of acute illness.

The role will include working as part of a multidisciplinary team, including GPs, Consultants, specialist doctors, Nurses, Pharmacy, Therapists, Social workers, Support Workers and other colleagues within Local Authority. We also work closely with independent providers of care providing training around medication.

The ICSD Pharmacy team also works closely with Community Pharmacy, ICB and the acute pharmacy team at Bolton NHS Foundation Trust. We recognise that effective collaboration is the cornerstone to the safe and effective use of medicines and therefore more positive patient outcomes.

The objective of the service is to offer patients the best opportunity to maximise their potential for regaining independence, and therefore relies on an integrated and committed multidisciplinary team to deliver the shared goal.

Main duties of the job

The purpose of this role is to work as part of a multidisciplinary team to:

  • Provide better co-ordinated care for these patients
  • Identify the medicines management needs of the patients within the ICSD
  • Reduce hospital admissions / re-admissions by being proactive
  • Reduce patient anxiety relating to medicines usage following hospital admission
  • Improve medicine safety
  • Improve medication compliance
  • Reduce medication wastage
  • Avoid duplication of use of services
  • Support the delivery of other services within ICSD

Detailed job description and main responsibilities

  • To provide specialised pharmaceutical advice and education to patients/carers and other health professionals 
  • Advise on therapeutic drug level monitoring for specific drugs
  • To support safe and cost effective delivery of pharmaceutical services  in bed based services and domiciliary care 
  • To provide assessment of patients ability to self-medicate and offer advice and guidance on self-management strategies for individuals
  • To carry out follow up domiciliary visits to patients where reviewing patients’ medicines and recommending processes to improve medicines optimisation ensuring all medicines are fit for purpose and appropriate
  • Ensure medicines related incidents are reported ensuring that appropriate learning is implemented
  • Assist in providing clinical training and supervision of pharmacy staff and other healthcare professionals

Essential criteria
  • Vocational Master’s Degree in Pharmacy
  • Registered as a Pharmacist with The General Pharmaceutical Council (GPhC).
  • Confident when dealing with staff of other disciplines at all levels of seniority & in a variety of disciplines
  • Good all round clinical knowledge and ability to use research findings to develop clinical services / patient care.
Desirable criteria
  • Post graduate diploma in clinical or community pharmacy.
  • NMP
  • Experience of root cause analysis and risk management
  • Demonstrable leadership skills & ability.
