Job overview

We are looking for an enthusiastic individual to join our friendly Intermediate Care Team.

This is a full time role (37.5 hours) to be worked Monday to Sunday.

The post holder will offer support to the qualified therapists across the bedded areas and Intermediate Care within the Wigan borough and may also be required to support the New Intermediate Care at Home team service within the community settings. 

You must hold either the NVQ Level 2 in Health & Social Care (from a recognised training provider) or the NHS accredited Care Certificate. We cannot accept the certificates from the short online courses as they are not NVQs.

It is essential for this role that you have the ability to independently travel across the borough to assess, formulate therapy plans and deliver therapy interventions for patients in their own place of residence.

Main duties of the job

The aim of the bedded areas is to prevent hospital admission or facilitate an earlier discharge from hospital to enable a patient to maximise their potential  to achieve a level of  function which is safe for them to return home or to a place of their choice.

Ideally the candidate will have experience working with patients with a variety of medical, functional and social  problems along with strong communication and team working skills.

Detailed job description and main responsibilities

Duties and Responsibilities

  • To assist in information collection for patients as requested by the Therapy Staff
  • To contribute to the overall Therapy process by carrying out routine assessments and procedures within agreed competency levels
  • To be responsible for preparation of materials and equipment in order to carry out treatment sessions
  • To follow treatment plans as prescribed by the Therapists identifying and selecting activities that meet treatment goals
  • To work independently in a community or clinical setting with an assigned caseload under the direction of a qualified Therapist
  • To be responsible for the selection, fitting and checking of equipment following a broad prescription from a therapist.
  • To keep accurate and clear patient treatment records in line with Trust Policies and Therapy standards
  • To maintain timely statistics and other records as requested by the community REACT Service

  • To establish therapeutic relationships using negotiating and reasoning skills in order to complete treatment activities.

Responsibility for Finance

  • To identify deficits in resources required to carry out the job and bring to the attention of the senior Therapy staff
  • To be responsible for the monitoring of materials and equipment and bring defects for deficits to the attention of Senior therapy Staff
  • Adhere to financial policies.

Responsibility for Human Resources

  • To attend regular meetings e.g. staff meeting, support and development groups
  • To participate in in-service training
  • To support junior staff as requested by Occupational Therapists.

Responsibility for Health & Safety

Compliance with the Health & Safety at Work Act 1974 – the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions. 

Responsibility for Teaching

  • To assist Therapy staff in induction and training of new staff and education of therapy students
