We are looking for motivated and qualified staff with the Specialist Practitioner Qualification, who will be a part of developing and driving our integrated approach to care, so that we can fully embrace and utilise theses benefits to provide the best outcomes for our patients. 

As part of LSCFT you will be supported to professionally develop and encouraged to continuously learn whilst considering new ways of working. The Community Specialist Practitioner will be a key member of the senior nursing and leadership team, responsible for providing the very best care to the patients in their own homes.  In addition you will be part of leading a team who are patient focused with excellence at its core, maintaining and improving professional standards and developments of the future workforce.

Main duties of the job

Applicants will need to demonstrate their competency and experience in line with the person specification and will be required to work flexibly across a seven day rota.

To provide professional leadership to a District Nursing Team and to facilitate a culture of learning and reflective practice within that team.

To assume continuous responsibility for appropriate deployment and supervision of staff for the effective management of a defined caseload.

To provide clinical leadership in the assessment, planning, implementation and evaluation of high quality evidence based nursing care to patients in their own homes and other community settings, to ensure that the care needs of the caseload are met. 

To provide direct clinical care to patients with identified specialist or complex needs.

To work to a defined shift pattern with a service responsible for the provision of care 24/7 365 days per year.
