Job overview

We have an opportunity for a Community Support Worker to take up a position within Wrightington, Wigan and Leigh Teaching Hospitals NHS Foundation Trust District Nursing Team within Leigh service delivery footprint.

An exciting opportunity has arisen for a highly motivated individual to join our busy team of dedicated district nurses within the Ashton, Wigan and Leigh locality. This could be for you if:

  • You have excellent communication skills;
  • You are able to work alone;
  • You have experience of working in community and managing own caseload;
  • You have experience administering insulin/eyedrops/basic dressings and venepuncture;
  • You have access to a car with suitable business;
  • You hold Level 2 Health & Social Care or NHS Care Certificate (we cannot accept certificates from the short online courses as these are not NVQs)

This is a full time role (37.5 hours) to be worked across a 7 day week.

As the role involves visiting patient homes & clinics across the Trust every day, postholders must state in their application if they have access to a car with suitable business insurance. If you do not mention this in your application, you will not be considered for the post.

An Enhanced DBS with Adults' Barred List check will be obtained during pre-employment checks.

Main duties of the job

We are looking for an individual with a passion for community care who must be able to work effectively within a team environment and independently organise and prioritise their individual workload.

Main Duties

  • To assist in the delivery of Nursing care to patients/carers in their homes or clinic setting as delegated by the team coordinator/team leader.
  • To undertake the extended role of a qualified NVQ support worker after appropriate Trust training.
  • To communicate effectively and appropriately with all team members and other agencies.
  • To undertake delegated duties which facilitate the smooth running of the district nursing team.

Based in the Community division so you will need to be able to travel to Ashton, Wigan and Leigh.

Candidates will be asked to present original qualification certificates to the interview panel.

Detailed job description and main responsibilities

  • To undertake the delivery of nursing care as delegated by Team Coordinator/Team Leader.
  • To undertake the extended role of a qualified NVQ level 3 practitioner after appropriate Trust training.
  • To undertake clinical procedures as delegated by the Team Coordinator/Team Leader and undertake invasive clinical procedures with set criteria, e.g. catheterisation, administration of rectal preparations.
  • To provide advice and health promotion to patients/carers as appropriate.
  • To undertake clinical observations.
  • To obtain specimens as clinically indicated and as requested.
  • To accurately document patient care in accordance with Trust policy.
  • To inform Team Coordinator/Team Leader of changes in the patient’s condition.
  • To take delegated responsibility for maintaining stock control and secure storage of stock.
  • To undertake non-direct patient related activities as delegated by the Team Coordinator/Team Leader, e.g. ordering of stationery.
  • To participate in audit activity as a requirement of practice.
  • To input information into the computer system.
  • To participate in the identification of personal and professional development needs through team objectives and personal development plans.
  • To participate in the staff appraisal scheme.
  • To undertake mandatory training in accordance with Trust policy.
  • To understand the responsibility of the role and justify decisions made in practice.
  • To actively seek guidance and appropriate learning and development opportunities.
  • To report sickness and absence in line with Trust policy.
  • To adhere to the Trust policies and procedures.
  • To comply with health and Safety policies to meet the requirements of the Health and Safety at Work Act.
  • To ensure prompt recording and reporting of accidents/incidents.

To organise own daily workload following delegation of daily duties.

Communications and Key Working Relationships

  • To use effective interpersonal skills when communicating with patients in particular when the atmosphere is hostile antagonistic or highly emotive.
  • To communicate with members of the primary health care team and multi agency’s to provide high quality holistic care to meet patient’s needs.
