Support worker required in the Health Visiting team. This is a permanent post working 34.5 hours Monday to Friday. Patient home visits may be required post holders must be able to travel independently across Trust. 

An Enhanced DBS with Children’s and Adults' Barred List checks will be obtained during pre-employment checks.

To support and assist the Health Visiting  and School Nursing Team within a range of settings, including Clinics, Health Centres, Schools, Start Well centres and other Community settings, whilst planning and prioritising their own workload following discussion with the Specialist Practitioner: Health Visitor/School Nurse.

To perform admin duties delegated by the Specialist Practitioner: either independently, without direct supervision or as part of a team, within a designated area and across the Trust as required in accordance to Trust and Health Visiting/School Nursing  Procedures and Guidelines.

To contribute to developing and maintaining good relationships with parents, Clinic staff, other disciplines and voluntary agencies: acting in a professional manner to ensure each contact with the Health Visiting/School Nurse service is both positive and informative.

To assist and contribute in the promotion of the positive health of children and families, aiming to prevent physical and psychological ill health.

Duties and Responsibilities

Key results from the job holder

  • To assist the Specialist Practitioner: in preparation for Well Baby Clinics delivered in Health Centres, GP surgeries, Start Well centres and community environments.
  • Measure children’s weights, record on electronic records and plot centiles, after the completion of training and assessment of competence.
  • Maintain stock levels of consumables and check equipment is in working order.
  • Assist Specialist Practitioner: Health Visitor in providing a health promotion service for children and families.
  • Undertake Health Visiting service clerical and administrative duties in accordance with Policy, Procedures and Guidelines. Maintain confidentiality of information and ensure safe custody of records either paper or electronic patient records.
  • Ensure accurate records are kept adhering to the Trusts Record Keeping Policy.
  • Communicate effectively in the receiving and providing of written, non-verbal and verbal information.
  • Maintain and adhere to relevant confidentiality, Data Protection Trust Policies applicable to role.
  • Report complaints, accidents and untoward incidents according to Trust Policy.
  • Identify and report any risk management issues.
  • To undertake and complete SystmOne activity as per Organisation and Service guidelines.
  • Develop Information Technology (IT) skills in line with Trust Information and Technology Strategy.
  • Observe rules governing health and safety as stipulated by the Trust Safety Policy.
  • Assist in the induction programmes of newly appointed Support Workers.
  • Contribute to facilitation and delivery of practice placements for students.
  • To have knowledge, understanding of Safeguarding Policy, Procedures & Guidelines and the importance of relaying information and referring to the Specialist Practitioner: Health Visitor.
  • Undertake Organisation and Service Specific Mandatory Training
  • Attend relevant training and update sessions in order to maintain and enhance knowledge and skills related to Health Visiting Core Service Delivery. 
  • To contribute to the development of new or revision of Health Visiting Procedures, policies and guidelines.
  • To participate and adhere to the Trusts Personal Development Review policy.
  • Contribute to Trust and Service Specific Audits and Benchmarks.
  • Contribute to the evaluation of Health Visiting Core Programme delivery.

Responsibility for Health & Safety

Compliance with the Health & Safety at Work Act 1974 – the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
