Job overview

We are looking for an enthusiastic, experienced and motivated pharmacist to join our team as Advanced Specialist Pharmacist – Homecare on a 12 month fixed term maternity cover basis.

This is an excellent opportunity for a pharmacist with strong clinical, governance and service improvement experience to take on a senior leadership role within a complex and high-profile area of medicines optimisation. The post holder will provide specialist pharmaceutical leadership for the pharmacy homecare service, supporting safe, effective, timely and patient-centred care for patients receiving medicines through homecare pathways.

Working closely with pharmacy, clinical teams, procurement, finance, contracting colleagues and external homecare providers, you will support robust governance, contract monitoring, risk management, service development and quality improvement across homecare medicines services. 
 

The role will provide continuity and senior support to the homecare service during a period of maternity leave, while also offering the successful candidate valuable experience in homecare medicines governance, high-cost medicines pathways, supplier liaison and cross-organisational working.

The post is based at Leicester General Hospital, with flexibility required to work across University Hospitals of Leicester NHS Trust sites, including Leicester Royal Infirmary and Glenfield Hospital.

Main duties of the job

The post holder will support the leadership and ongoing development of the Trust’s pharmacy homecare service, ensuring that medicines supplied through homecare pathways are managed safely, effectively and in line with Trust, regional and national standards.

Key responsibilities will include:

Providing expert pharmaceutical advice on homecare medicines governance, prescribing processes, pathway design and service delivery.

Supporting clinical teams with the implementation and review of homecare medicine routes, particularly for high-cost and specialist medicines.

Maintaining and strengthening governance arrangements for homecare services, including incident review, risk management, audit, documentation, assurance and compliance monitoring.

Working with procurement, finance, contracting colleagues and homecare providers to monitor contract performance, activity, expenditure, invoicing issues and opportunities for improved value.

Investigating, escalating and helping to resolve homecare service issues, delays, complaints, supply problems and quality concerns.

Developing, reviewing and maintaining homecare-related policies, standard operating procedures, guidance and training materials.

Supporting audit, reporting and quality improvement activity to improve patient safety, experience, efficiency and service performance.

Providing leadership, supervision, education and expert support to pharmacy staff and multidisciplinary colleagues involved in homecare medicines services.

Detailed job description and main responsibilities

The post holder will provide senior professional leadership for the pharmacy homecare service on a 12-month fixed term maternity cover basis. The role will support continuity of service leadership, governance and operational oversight for patients receiving medicines through homecare arrangements.

The post holder will work collaboratively with pharmacy teams, clinical services, procurement, finance, contracting colleagues, external homecare providers and wider system partners to maintain robust homecare medicines pathways. This will include supporting patient onboarding, continuity of supply, escalation processes, communication arrangements and resolution of operational or quality concerns.

A key part of the role will be to maintain and strengthen governance and assurance across homecare services. The post holder will lead or contribute to incident review, risk management, audit, service reporting, supplier governance, compliance monitoring and the implementation of medicines safety and quality standards. They will also support the development, implementation and review of policies, standard operating procedures, training materials and service documentation relevant to homecare medicines.

The role will contribute to the oversight of homecare medicines expenditure and service performance, including review of high-cost medicines use, contract activity, invoicing issues, financial discrepancies and opportunities for improved value and efficiency. The post holder will support procurement, contract review and service improvement processes, helping to ensure that homecare medicines services remain safe, sustainable and responsive to patient and organisational needs.

The post holder will provide education, supervision and specialist advice to pharmacy, nursing, medical, administrative and operational colleagues on homecare governance, prescribing processes, patient pathways and medicines supply arrangements. They will be expected to communicate complex information clearly and effectively with patients, carers, clinical teams and external providers, supporting timely escalation and resolution of service issues.

Applicants should have significant recent post-registration hospital pharmacy experience, excellent communication and organisational skills, and experience of service development, governance or medicines optimisation. Experience of homecare services, high-cost medicines, specialist pharmacy services, audit, reporting or investigation of service issues would be particularly valuable.

This fixed term post would suit a pharmacist looking to broaden their senior leadership experience in medicines homecare, governance, high-cost medicines and cross-organisational service delivery, while making a meaningful contribution to a specialist service that directly supports patient care.
