Job overview

The Renal Community Nurse is responsible for delivering high-quality, patient-centred care to adults receiving home dialysis therapies, including Peritoneal Dialysis (PD) and Home Haemodialysis (HHD). Working as part of the Renal Community and Home Therapies Team, the post holder will support patients to safely manage their treatment at home through education, training, assessment and ongoing clinical support.

The post holder will undertake home visits, outpatient clinic activity and virtual patient reviews, monitoring patients’ clinical condition and responding to complications associated with renal replacement therapies. They will work collaboratively with the multidisciplinary team to promote independence, improve patient outcomes and support patients to maintain dialysis treatment within their home environment.

The role includes supporting service development, quality improvement, audit and research activity where appropriate. The post holder will contribute to maintaining high standards of clinical care, patient safety and governance whilst acting as an advocate for patients and their families throughout their renal care pathway.

The role involves regular travel to patients' homes, community settings and Trust sites. Therefore, the post holder must hold a valid UK driving licence and have access to a vehicle for business use. Mileage expenses will be reimbursed in line with Trust policy.

Main duties of the job

  • Delivery of effective health care within the organisation.

  • Co-ordinate, organise and prioritise the work of the nursing team on a day-to-day basis.

  • Supervise others and participate in their education and development. 

  • Maintain effective and efficient communication with the Unit Manager and MDT.

  • Adhere to the UHL Uniform policy at all times.

  • Ensure own compliance and NMC Professional re-registration requirements. 

  • Promote patient dignity, equality, diversity and rights.

  • Ensure own and all nursing practice is, at all times, in accordance with the NMC Code of Professional Conduct and raise any concerns with the Unit Manager.

  • Use health-related information only for the purposes for which it is intended and in accordance with the Data Protection Act.

  • Ensure confidentiality is maintained at all times and information relating to patient/users and personnel is used only in connection with authorised duties.

  • Move and handle patients and goods in ways that promote the health of the patient and care team and that are consistent with legislation.

  • Responsible for the maintenance of a clean and safe environment adhering to IPC protocols.

Detailed job description and main responsibilities

  • This role requires the ability to develop strong collaborative relationships across organisational boundaries to support patients receiving home dialysis therapies and to contribute to the ongoing development of renal services.

  • Deliver high-quality, evidence-based nursing care to patients undertaking home dialysis therapies, including Peritoneal Dialysis (PD) and Home Haemodialysis (HHD). 

  • Assess, plan, implement and evaluate individualised programmes of care for patients within their home environment and outpatient settings. 

  • Monitor patients' physical, psychological and social wellbeing, escalating concerns appropriately and coordinating care with the wider multidisciplinary team.

  • Provide ongoing monitoring, troubleshooting and support for patients established on home dialysis therapies. 

  • Undertake home visits, telephone reviews and virtual consultations as required to ensure safe and effective treatment delivery. 

  • Identify and address barriers to successful home therapy, promoting independence and patient self-management.

  • Work collaboratively with nephrologists, specialist nurses, dietitians, pharmacists, psychologists, social workers and community healthcare professionals. 

  • Contribute to multidisciplinary team meetings, patient reviews and discharge planning processes. 

  • Facilitate effective communication between primary, secondary and community care providers.

  • Maintain accurate, timely and comprehensive patient records using electronic and paper-based documentation systems. 

  • Participate in clinical audit, quality improvement initiatives and service evaluations to support continual improvement. 

  • Ensure compliance with Trust policies, professional standards, clinical guidelines and regulatory requirements.

  • Act as a role model for professional practice and demonstrate Trust values at all times.

  • Support students, new staff and colleagues in their learning and development.

  • Contribute positively to team objectives and service priorities, demonstrating flexibility to meet service needs.

  • Promote a positive patient experience through compassionate, person-centred care. 

  • Support patients and families to make informed choices regarding renal replacement therapy options.

  • Assist with patient recruitment, data collection and implementation of research findings into clinical practice where appropriate.

  • Promote an evidence-based approach to patient care and contribute to the dissemination of learning within the team.
