The DLN Cluster has been established to support the wider health and social care needs of the population across Derby & Derbyshire, Lincolnshire and Nottingham & Nottinghamshire and will focus on reducing health inequalities, moving care closer to home, and ensuring long-term financial and service sustainability. Its core framework targets prevention, integrated neighbourhood health models, and managing high-risk multimorbidity.

Job overview
This post is essential to the effective delivery and transformation of the NHS Derby and Derbyshire, NHS Lincolnshire and NHS Nottingham and Nottinghamshire Cluster ICBs All Age Continuing Care/CHC/S117/ Services moving forward. The post is open to those with either a current Clinical or Professional Qualification and will work as the formal Deputy to the Director of Quality, Safeguarding, CHC and Partnerships in respect of these Services. You will have significant experience of these specialist areas of work and experience of significant service transformation along with a collaborative and compassionate style of leadership and management.
Main duties of the job
The specific responsibilities of this post are to:
- Lead CHC operational strategy and transformation programmes for the 128 months following appointment developing forward plans for following years
- Manage a significant budget ensuring financial probity and delivery of agreed savings across the Cluster ICBs
- Deliver a new model of operational delivery across the cluster that maximises integration and improvement.
- Oversee all aspects of delivery including appeals, complaints, FOIs and SARs
- Lead S117 oversight and joint funding processes
- Provide strategic leadership for market management and brokerage
- Ensure compliance with national frameworks and statutory guidance
- Lead CHC teams across the three ICB footprints
- Manage change programmes driving efficiency, consistency and high-quality outcomes
- Implement redesigned models of CHC and individual care packages
- Oversee CHC risk, performance, and reporting

Knowledge, Training and Experience
Essential criteria
- Member of relevant professional body either Clinical or Professional with active registration
- Relevant Master's level qualification or equivalent experience working at a senior level in highly complex health or social care services
- Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master’s level equivalent
- Evidence of continued professional development and professional leadership
- Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this on engagement
- Should have an appreciation of the relationship between the Department of Health, and individual provider and commissioning organisations
