Job overview

This is an exciting opportunity to join a dynamic Rehabilitation Medicine service delivering specialist care across inpatient, outpatient, and community settings. Based primarily within the Rehabilitation Unit at Lincoln County Hospital, the post holder will play a key role in providing high-quality, patient-centred care for individuals with complex rehabilitation needs.

The successful candidate will manage patients within the specialist rehabilitation unit and deliver outpatient clinics in areas such as Spasticity Management, Acquired Brain Injury, and Neurological Rehabilitation. Working closely with multidisciplinary teams, they will assess referrals from across the Trust and contribute to the development of seamless rehabilitation pathways.

The role also includes undertaking community and home visits to support patients throughout their rehabilitation journey, promoting effective discharge planning and ongoing care.

In addition, the post holder will contribute to the education, supervision, and development of junior medical staff, while actively participating in clinical governance, audit, service evaluation, and quality improvement initiatives. This is a varied and rewarding position that offers the opportunity to help shape and develop rehabilitation services across the Trust, making a meaningful impact on patient outcomes throughout the region.

Main duties of the job

·  Manage an independent clinical caseload, providing supervision, support and guidance to junior medical staff and members of the multidisciplinary team as appropriate.

·  Promote a culture of safety by ensuring the wellbeing of patients, colleagues and self, and supporting junior staff in meeting their patient safety and risk management responsibilities.

·  Work in line with Trust policies, procedures and professional standards, contributing to service development and the implementation of best practice.

·  Maintain high standards of professional practice, delivering safe, effective and patient-centred care.

·  Uphold confidentiality of all patient, staff and organisational information in accordance with Trust policies, GDPR and professional standards.

Detailed job description and main responsibilities

Inpatients

The inpatient element of this role is based primarily within the Ashby Rehabilitation Unit, which provides 18 specialist rehabilitation beds (15 Level 2a and 3 Level 2b). The unit accepts referrals from major trauma centres and acute hospitals across the region for patients requiring specialist neurorehabilitation following conditions such as traumatic brain injury, brain tumour surgery and disorders of consciousness.

The post holder will review referrals presented through the weekly Specialised Rehabilitation Patient Categorisation (PCAT) meeting and undertake assessments of patients requiring specialist rehabilitation at Lincoln County Hospital, Pilgrim Hospital and Grantham Hospital. They will also contribute to the Rehabilitation Unit on-call rota.

Outpatients and Community Services

The successful candidate will deliver specialist Neurorehabilitation, Brain Injury and Spasticity clinics across multiple hospital and community locations, including Louth, Gainsborough and Spalding.

The role also includes undertaking home visits where clinically appropriate, ensuring patients with complex needs or mobility challenges can access specialist rehabilitation support.

Clinical Duties, Administration and Leadership

The post holder will be responsible for managing their clinical and non-clinical workload effectively, providing high-quality, patient-centred care and making informed clinical decisions within a multidisciplinary team environment.

They will maintain strong working relationships with colleagues across the Trust, General Practitioners, regional trauma centres and partner organisations to support coordinated patient care. The successful candidate will also contribute to service development, operational policy implementation and continuous improvement initiatives.

Key responsibilities include:

  • Managing the clinical care of patients under their supervision.
  • Undertaking associated administrative and service management duties.
  • Participating in clinical governance, audit and quality improvement activities.
  • Supporting service evaluation and the ongoing development of rehabilitation services.
  • Attending departmental, multidisciplinary and organisational meetings.
  • Contributing to the teaching, training and supervision of junior doctors and other healthcare professionals.

This varied and rewarding role offers the opportunity to work across inpatient, outpatient and community settings while helping to shape and develop specialist rehabilitation services across Lincolnshire.

Qualifications

Essential criteria
  • Current valid GMC registration
  • MRCP or equivalent
  • Entry on the General Medical Council (GMC) Specialist Register in Rehabilitation Medicine via one of the following: a) Certificate of Completion of Training (The proposed CCT date must be within 6 months of the interview) b) Certificate of Eligibility for Specialist Registration (CESR) c) European Community Rights
Desirable criteria
  • Advanced Life Support certificate
  • Higher research qualification

Clinical Experience and Management

Essential criteria
  • Broad clinical experience in Rehabilitation Medicine, including inpatient, outpatient and community rehabilitation services.
  • Demonstrable ability to manage complex rehabilitation patients independently and provide senior clinical leadership within a multidisciplinary team
  • Experience in the assessment and management of patients with acquired brain injury, neurological disabilities and spasticity, including the use of specialist rehabilitation interventions
  • Evidence of understanding of the role of clinical management
  • Ability to plan strategically and to exercise sound judgment when faced with conflicting pressures
Desirable criteria
  • Evidence of developing a high quality, high-performance service

Clinical Effectiveness

Essential criteria
  • Evidence of contribution to effective clinical audit and clinical risk management

  • A record of quality improvement or clinical audit relating to the care of older patients

Teaching, Traning and Research

  • Experience of undergraduate teaching and post graduate training for doctors and other disciplines
