Job overview

We are looking for a  Senior Health Care support worker to join our experienced team within the Louth & Wold Community Nursing team 

Located within the grounds of Louth Hospital  the community nursing team covers a wide geographical area.  You will visit housebound patients delivering wound care, medication, palliative care from a care plan under the direction of a registered clinician.    This is a very rewarding job for anyone interested in supporting patients at home.  You will work autonomously  once induction has been completed ( pool car available) . Previous experience in community working would be advantageous. The team welcomes  in site visits 

 

Main duties of the job

The post holder will actively deliver  care that meets the needs of patients at home or within a  care setting  under the direction of a qualified Nurse  You will working within the Trust's policies and procedures at all times.  

The post holder will record patient information and carry out a range of delegated clinical health care duties including  nursing  care ensuring that patients /clients receive safe, individualised and effective care which supports health and wellbeing and promotes self- care where appropriate and possible.

The post holder may also be expected to perform a range of non-clinical duties as requested including ordering of stores, audits, and  attendance at allocated meetings.

Detailed job description and main responsibilities

The successful candidate will have the following:

Commitment to Continuous Professional Development (CPD) and lifelong learning with the ability and willingness to undertake competency-based training both initially and as ongoing requirement for the post to meet service requirements.
Previous experience working and communicating with the public.
Good computer/ IT skills
Clinical experience in a healthcare setting undertaking a range of delegated clinical tasks supporting patients.
Ability to work in a team.
Ability to communicate with a variety of different people, adapt approach accordingly and build rapport.
Ability to follow written and verbal instructions.
Ability to use initiative and be aware of limitations.
Ability to switch between activities required by the service.
Ability to work to strict deadlines with accuracy.
Ability to stay calm in the event of the unexpected and under pressure.
Ability to deal sensitively with distressing, emotional situations.
Ability to work unsocial hours including  shift work and bank holidays.

Qualifications

Essential criteria
  • Good standard of general education NVQ level 3 or ability to demonstrate equivalent knowledge, skills or experience in health / social care environment
  • GCSE grade c in English and Maths
Desirable criteria
  • ECDL
  • Food hygiene certificate

Experience

Essential criteria
  • Experience within a caring environment.
  • Experience of working with people and dealing with members of the public.
  • Can demonstrate attitude and aptitude appropriate to the post. Ability to relate previous experience to the role.
Desirable criteria
  • Experience of working within a multi disciplinary team.
  • Previous experience delivering care in a community setting.
  • Experience of working in a rehabilitation environment.

Skills

Essential criteria
  • Has insight into the needs and circumstances of vulnerable people.
  • Must be able to undertake all aspects of the role whilst working a lone worker.
  • Effective communication skills, both verbal, written and electronic.
  • Ability to forge effective working relationships, with both patient s and their relatives and colleagues.

  • Knowledge of rehabilitation.
