Job overview

We are looking for an enthusiastic person to join us in the older people and frailty team in Spalding. Our team support patients over the age of 65 with severe and enduring mental health diagnoses and provide an all-age memory assessment, diagnosis and treatment pathway.

This job role will focus on our memory pathway, and the post holder will be supporting our patients who already have a diagnosis of dementia. We are based at the Johnson Hospital in Spalding, a modern purpose-built hospital with free staff parking.

Main duties of the job

The main duties of this role will be to support patients newly diagnosed with dementia when starting medication by monitoring side effects and responses and reporting these back to the medical team. You will also be responsible for yearly checks of dementia medication with patients and their carers and then communicating any concerns back through to the medical team. Training and support will be given. The candidate will require excellent communication and organisational skills to undertake this role and will also need to have a good understanding of basic computer skills, as our clinical notes are all digital.  You will be expected to investigate information from several clinical systems and sources.

Training on these systems would be provided. Ideally the candidate will have a broad understanding of dementias, and it would be a suitable role for someone who has already worked within the care sector or with patients who have a dementia diagnosis.

Person specification

Qualifications & Experience

Essential criteria
  • NVQ level 3 qualification or equivalent in a health/social care related subject.
  • Training in mental health awareness or other training relevant to this post.
  • Previous relevant experience working with people with mental health problems at a nursing assistant grade or equivalent within a health and social care setting.
  • Experience in providing support for people in the community.
  • Experience of working with people with mental health needs.
Desirable criteria
  • Willingness to engage in training provided by the division and professional training to support this role
  • Previous experience within a dementia care setting or within dementia community services/ third sector providers.
