Lincolnshire Community Health Services NHS Trust (LCHS) and United Lincolnshire Hospitals NHS Trust (ULHT) have come together in a Group arrangement, with the goal of improving the care provided to patients across the county.
This will not constitute a formal merger of the two organisations, but will bring the Trusts together under a single Board and Executive Leadership Team, with the goal of improving the care that is provided to patients both in the community and in hospitals across Lincolnshire. Both Trusts will retain their separate statutory names and legal obligations.
The Group will be known as Lincolnshire Community and Hospitals NHS Group (LCHG).

Job overview
Are you a newly qualified or existing Band 5 physiotherapist wanting to gain experience of community working. Within LCHS we offer an exciting opportunity for a B5 Physio to join our team of rotational physiotherapists who are currently working in our wide range of services which can include:
- Adult Therapy and Transitional Care
- Community Hospitals
- Lincolnshire Stroke Service
- Cardiac Rehabilitation
- Pulmonary Rehabilitation
- MSK outpatients
- Discharge to Assess
Services are county wide across Lincolnshire and base dependant upon each rotation. The first rotation for these 2 posts will likely sit within Adult community therapy
Our staff see people in their own homes, care homes, community rehab wards and clinics, working with individuals to support them to be the best they can be; following the Home First principles and providing care closer to home. This will provide you with a sound base of knowledge and experience to take forward in your career.
LCHS is committed to developing our staff with their training and learning and offer a wide range of opportunities to enhance your skills. If you are a newly qualified physiotherapist, we offer a robust preceptorship programme which is approached within an MDT setting.
With a strong leadership structure to support your employment within LCHS, you will have mentorship, regular supervision and guidance and peer support available to you.

Main duties of the job
The teams offer specialist holistic therapy assessments and intervention with the aim of maximising function and independence and therefore reducing the requirement for care services and supporting the ethos of physiotherapy. Care planning and patient-centred goal setting is a fundamental part of the person’s journey and integration with social care teams, nursing teams, and other stakeholders is essential. The teams also have a proactive approach with emphasis on self-management of conditions, and empowering people to live well for longer.
As a band 5 Physiotherapist, you will have the opportunity to assess people with a wide variety of conditions such as post-surgery and post-stroke rehabilitation, falls and frailty and deteriorating long term conditions e.g. Parkinson's Disease and COPD on your rotations.
There will be an expectation that you will need to travel around the county on some rotations.

- Current Health and Care Professions Council (HCPC) Registration
- Basic IT skills
- Ability to travel
- Ability to successfully complete a 6 month preceptorship for newly qualified or return to practice physiotherapists and competency framework
Desirable criteria
- Member of Chartered Society of Physiotherapy
