We are seeking to appoint a committed, highly motivated, enthusiastic, and skilled Band 7 Advanced Primary Care Nurse to join our dynamic, integrated multi- disciplinary Rapid Response/ Urgent Community Response Team. The Bexley Rapid Response Team were part of the South East London Accelerator Programme. The team are delivering the essential elements of the Urgent Community Response (UCR) pathways, this includes responding to urgent referrals within 2 hours, providing falls pick up service and virtual ward.
Please note this post requires a Qualified Adult Registered Nurse (RNA, RN1, RN2).
In this exciting role you will be an integral part of the team developing new pathways to provide Same Day Emergency Care in the community alongside other Adult Community Services, Social Care, Acute Trusts and Primary Care colleagues.
The team are based at Goldie Leigh, Lodge Hill Abbey Wood, and operates 7 days a week, on an 8am-8pm basis, focusing on rapid 2-hour face to face assessments to patients who are in a health or social care crisis. The team offers short-term intervention(s) typically lasting up to 5 days.
Main duties of the job
The post holder will conduct comprehensive advanced nursing assessments for patients within the community settings, post holder will be expected to undertake independent nurse prescribing within the community and demonstrate complex clinical reasoning and risk assessment. The post holder will hold the skills to guide appropriate nursing intervention to meet the needs of patients referred to the team, ensuring the highest possible standards of practice with a strong emphasis on Falls and Frailty, early supported discharge, breast drain care and Outpatient Parenteral Antimicrobial Therapy (OPAT). The post holder will also have the opportunity to work closely with our Community Consultant Geriatrician.
The team work directly with LAS, 111, GPs and act as a liaison between Emergency Departments, community, and care home settings, by identifying the most appropriate onward referrals and initiating integrated care pathways, ensuring seamless provision of care and the instigation of on- going case management of long-term condition management.
Due to the requirement to be able to travel across the Borough in a timely manner, a full driver’s licence is essential as well as access to a car for work purposes. There is a salary sacrifice scheme lease car scheme available for all Oxleas employees.

Detailed job description and main responsibilities
- The post holder will be an autonomous nurse practitioner able to asses, interpret clinical findings and implement care to patients.
- When appropriate undertake evidence based independent nurse prescribing when required in line with clinical assessment and reasoning.
- To analyse and manage highly complex changing health and social care situations, to prioritise unscheduled calls at short notice due to sudden changes in clinical presentation or social care requirements.
- Undertake joint working with other members of the Rapid Response team to maximise patient assessment and care planning
- To ensure key performance Indicators and quality information is maintained and produced
- Receive direct referrals from General Practitioners (GP’s) and London Ambulance Service (LAS) 111 and work in conjunction with them to prevent unnecessary A and E attendance
- Maintain effective multidisciplinary communication with other members of the team and the wider clinical network to ensure that client’s needs are met.
