An exciting opportunity has arisen for a B8a Advanced Practitioner for SEND to join CDC nursing in the London Borough of Barking & Dagenham NELFT.  We are looking for someone who enjoys working as part of a skilled interdisciplinary team within our children’s service, working collaboratively with our partner agency for the benefit of children with special educational needs and disabilities and their families’/carers’.   The service supports babies’ children and young people in the borough across health visiting, school nursing and specialist teams, working collaboratively to ensure BCYP with additional needs (SEND) are assessed, diagnosed and supported appropriately to improve support and outcomes for their future. Working with our local partners to ensure the local offer is reflective of service provision that is responsive to needs.  This is an exciting time to join our children’s service, and drive innovation for BCYP with SEND forward.  This includes the development of neighbourhood MDT’s and our own Proactive care pilot for children which has made significant impact on children with complex health needs accessing their education. 

This is a full-time post with the opportunity for innovation in service provision working with the Consultant Nurse to ensure that Barking & Dagenham lead the way in meeting the needs of our diverse population.  Working on QI projects and co-production to ensure the local needs are the drivers for change.

The post holder will be a qualified Advanced Nurse Practitioner with the V300.  

  • Act as a role model for the wider teams and service
  • Support and manage the Specialist School Nursing Team
  • Be responsible for unertaking comprehensive health assessments, physical examinations, initiate referrals and investigations.  Working closely with the Consultant Paediatricians, ANP for ADHD and Consultant Nurse.
  • Be involved in establishing delivering and leadning a comprehensive service within community service for children with SEND.
  • Work with parents/carers and local stakeholders to deliver innovative and impactful interventions and strategies.

Starting with Us

NELFT places great importance on new starters being properly welcomed into the Trust.  All new starters will join the Trust on the first Monday of each month and undertake a comprehensive induction of up to two weeks, including mandatory training, systems training and the allocation of equipment.  As part of the process, new starters will also have the opportunity to meet the executive team, senior managers, and attend several drop-in sessions focusing on engagement, health and wellbeing, and key processes.  The induction will be held at our head office in Rainham, Essex.

Probationary Period

This post will be subject to a probationary period. 

High Cost Area Supplement - Outer London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part time).

Be a careing, compassionate and committted experenced registered nurse and advanced clinical practitioner.  Responsible for patients on thier caseload and will assess, manage, diagnose, treat, plan deliver and evaluate care.  Deomnstrating safe, competent clinical decision-making and expert care, ensuring the NMC code of professional conduct and the Trust values are embedded in practice.

Support the breaking of diangostic news ensuring the family are supported through the process with access to resources and are able to leave appointments safely.  

Support the Consultant Nurse in delivering on the children's agenda especially SEND.
