The Medical HSEP (Health Services for Elderly People) team, specialising in working with patients living with frailty, is excited to be able to offer a Band 6 Physiotherapy or Occupational Therapy post.  

You will be joining a friendly, well established integrated therapy service, working alongside experienced colleagues and supported by experienced occupational therapists and physiotherapists as well as an Advanced Clinical Practitioner in Frailty, and Clinical Lead therapist. The post will be based on the medical and HSEP wards, where you will be working alongside cohesive, supportive and forward thinking multidisciplinary teams.

Person-centred care and a supportive approach for our patients living with frailty are at the the centre of our philosophy.

Prif ddyletswyddau'r swydd

To manage a defined caseload, using evidence based/patient centred principles to assess, plan, implement and evaluate occupational therapy/physiotherapy interventions.
 To provide leadership for junior staff, as delegated by supervisor.
 To supervise occupational therapy/physiotherapy students on practice placements.
 To undertake all aspects of clinical duties as an autonomous practitioner whilst facilitating effective multidisciplinary care within relevant clinical areas.
 To work in an integrated therapy team including other physiotherapists,
occupational therapists, dieticians, speech and language therapists, therapy support workers and administration support.
 To provide an advanced standard of physiotherapy/occupational therapy assessment and treatment to inpatients without direct supervision. Access to advice and support from a team lead is available as and when required. Clinical work is routinely evaluated.
 To work with patient’s families or carers to provide an advanced level of support, education and expectation of the outcome of the inpatient stay.

To supervise, teach and appraise Band 5 physiotherapists/occupational therapists and support workers.

  • Understanding of clinical governance and its implication for services, including experience of quality issues and audit
  • Demonstrates a clear desire to work with patients living with frailty
