Job overview

The RFL Neurology Service is seeking a motivated individual to  join a team of patient navigators, responsible for supporting and maintaining administrative functions and patient pathways within the department for both our patients and clinicians.

The postholder will support patients through their hospital care journey, providing the patient with a named point of contact who will co-ordinate their care.

The successful applicant will need to work as part of a team within a dedicated speciality/patient group to ensure that all administrative functions for patients from the time of GP referral to the time of discharge are coordinated in line with specific care pathway instructions to facilitate the smooth running of the patients’ journey. Where a patient has multiple specialist needs, they will need to assist the patient in coordinating their pathways to reduce multiple appointments across the Trust. 

Main duties of the job

The  Neurology service is looking for a dynamic and resilient clinical pathway navigator to join our team.  This is a pivotal role within the team and will be supported by the Clinical Pathway Manager and Service Manager.  The successful applicant would be responsible for supporting service delivery and improvement within the department.
