Job overview

The CNS contributes to the provision of specialist palliative care in the acute hospital setting for patients facing life-limiting illnesses (within the remit of the multi- disciplinary team) accepting patient referrals to agreed criteria and managing their own caseload with the support of the Lead Nurse and Consultant Nurse. They will provide advice, education and support to patients, their carers and other health and social care professionals. They will contribute to the development and smooth running of the service. The post holder will work to agreed professional, departmental and Trust boundaries as required to provide patient centred seamless care and will take part in the 7 day / week service for hospital patients at Barnet Hospital.

Main duties of the job

To manage a caseload and provide an expert and autonomous nursing
service for patients with life limiting illness and potentially facing end of
life care issues, and support them and their families, using advance
clinical judgement and knowledge in palliative care.
 To lead and innovate nursing practice in palliative care on the Barnet
Hospital site
 To take part in a 7 day / week face to face clinical nurse specialist –
specialist palliative care service.
 To contribute to the weekly specialist palliative care MDT
 To design and deliver specialist palliative care education; and deliver end of life education. To support and mentor students in the team.
 To take part in the regular palliative care team programme of audit and
review of clinical practice. To develop research skills and contribute to
the development of multi-disciplinary research within palliative care.

We will offer continuing education, support, and development, to develop your skills as a clinical nurse specialist.

  • • Degree in Palliative Care / Oncology / Critical Care / Long term condition / other relevant speciality

  • • Masters Degree in relevant subject or willing to study to this level
