Occupational Therapist

Job overview
We are looking for a skilled and experienced Occupational Therapist to work within a well established and respected, adult multi-disciplinary community neurological team.
The successful applicant will be responsible for the assessment and treatment of adults with a variety of neurological conditions offering rehabilitation, disability management and end of life care. The service offers Early Supported Discharge for Stroke patients. The Occupational Therapy Service also offers a comprehensive splinting service and vocational rehabilitation. There is a strong emphasis on multi-disciplinary working especially within service development and team learning. The MD teams include Occupational Therapists, Physiotherapists, Speech and Language Therapists, Dietician, Clinical Neuropsychologist, and Clinical Nurse Specialist who are supported by Psychology, Admin and Therapy Assistants.
Main duties of the job
As a Neuro Occupational Therapist working in the community, you will provide specialist rehabilitation and support to individuals with long-term neurological conditions . Your primary role is to enable patients to regain and maintain independence in daily activities, improve functional abilities, and enhance their quality of life within their home and community settings.
Key Responsibilities:

Conduct detailed assessments of patients’ physical, cognitive, and psychosocial abilities and needs.

Develop and implement personalised rehabilitation plans focused on functional goals, adapting interventions to individual patient circumstances.

Provide practical training and support for activities of daily living (ADLs), including self-care, mobility, and home management.

To provide disability management which may include the prescription of assistive devices, adaptive equipment, and environmental modifications to promote safety and independence.

Work collaboratively with multidisciplinary team and wider organisations to coordinate care and ensure holistic treatment.

Offer education and support to patients, families, and carers to empower self-management and reduce caregiver strain.

Monitor progress through regular reviews and adjust treatment plans accordingly.
- Participate in discharge planning from hospital to community care, facilitating early and safe transitions.

Qualifications
Essential criteria
- • Diploma/Degree in Occupational Therapy
- • Registered to practice as Occupational Therapist with HealthCare Professions Council
- • Evidence of recent and relevant CPD
Desirable criteria
- • Attended Practice Educators’ Course
- • Attendance at recent post graduate courses relevant to the clinical field of neurology.
Experience
Essential criteria
- • Post qualification experience in any of the following areas neurology, neurorehab, stroke, Trauma, neurosurgery or community neuro work.
- • Working as part of a MDT
- • Ability to form a sound prognosis and advise other members of the MDT
Desirable criteria
- • Experience of working with support workers.
- • Experience of providing Vocational rehab.
- • Teaching experience in in-service programs
- • Involvement in Clinical audit and quality improvement
- • Experience of the research process
Skills
Essential criteria
- • Good assessment skills based on a sound clinical reasoning approach
- • Able to work harmoniously with a diverse workforce to deliver a quality service to people from different backgrounds by including and embracing their varied needs.
