Job overview

 The Older People’s Mental Health Community Mental Health Team work closely with GPs to ensure early identification of patients with mental health needs and facilitate timely referrals. Assessments will be provided by a multi-disciplinary team, ensuring carers and family are involved at the initial assessment and in care planning.

Through early diagnosis of mental disorders the delivery of evidence based therapeutic initial interventions (pharmacological and psychological) will be facilitated in a timely fashion. 

Support and information delivered in a person-centered way after the diagnosis to ensure the person with dementia and their carer are adjusting to the diagnosis and its consequences and are engaging in personal care plans and understand how to access other assistance.

Continuity of care across the pathway and integration at patient level with other care providers

 

Delivery of care in a considered, timely and co-ordinated manner

 

This integrated approach to the care of the elderly and the support of their carers, will be provided in partnership with local healthcare, social care and voluntary/ independent organisations.

Main duties of the job

  1. Assessment and treatment of functional and organic psychiatric disorders in the community as well as in Nursing/ Residential Homes in Bexley.
  2. Responsibility for the continuing care dementia units in Bexley with regular input and reviews.
  3. Facilitate early discharge planning and effective use of the inpatient units’ beds
  4. Preparing papers and presenting to the Multidisciplinary panel for placements in residential and nursing homes for older people known to the team
  5. Input to Multidisciplinary panel discussions for placement reviews in residential and nursing homes for older people known to the team
  6. As it is an integrated team  (integrated with social services)  there is social care input to the patients known to our team with regards their care packages, respite care and placements via the MDT panel.

Detailed job description and main responsibilities

            Assessment and management of patients in the community with primarily memory problems who meet the team’s inclusion criteria.

            Joint working, support and advice on the management of appropriate patients to other members of the Memory Team as well as to colleagues in Social Services

            To participate in care planning meetings

            To liaise with other parts of the service (especially the wards and IHTT when urgent assessments are needed) . To also liaise with GPs and non NHS organisations when indicated

            To cover for colleagues during planned absence and short periods of sick leave

            Supervision of junior medical staff

            To support with medical leadership and senior clinical support to the multi- disciplinary team.

            To undertake more complex capacity assessments and apply the Mental Capacity Act and Mental Health Act appropriately

            To deliver pharmacological treatment in accordance with NICE guidance.

            To be aware of current research and discuss participation in research programmes with patients
