Job overview
An opportunity has arisen within the CNWL NHS Foundation Trust, Community Health Services, for a Specialist Occupational Therapist to join our Staying Steady, falls prevention service. Due to the expanding and innovating project work, currently under development, we are further resourcing our team to carry out this exciting new work. Based in Bletchley, Milton Keynes, Staying Steady is a multi-professional team who undertake multifactorial risk assessments, provide enabling solutions using equipment prescription, advice, signposting and other treatment interventions to reduce falls risks for adults in Milton Keynes.
This post will provide an opportunity to further develop your assessment and treatment skills for people who have fallen or are at risk of falling. You will be working alongside experienced and specialist Physiotherapists, Occupational Therapists, Nurses, Rehabilitation Support Workers and Consultant, to provide coordinated assessment and goal orientated intervention to service users.
Main duties of the job
- To provide advanced assessment of patients who are at risk of falling or who have fallen, to develop and deliver individualised treatment programmes.
- To undertake telephone triage, multi-factorial assessments, equipment and other therapeutic input as part of the Multidisciplinary Team.
- To hold responsibility for own caseload.
- To undertake all aspects of clinical duties as an autonomous practitioner.
- To undertake evidence-based audit and research projects to further own and team’s clinical practice.

Detailed job description and main responsibilities
- To be professionally and legally responsible and accountable for all aspects of your own work including the management of patients in your care and that of students and assistants under your direct supervision. To ensure a high standard of clinical care for the patients under your management.
- To undertake the comprehensive assessment of patients, including those with a complex presentation/multi-pathologies; use advanced clinical reasoning skills and manual assessment techniques to provide an accurate assessment/diagnosis of their condition.
- To formulate, deliver and evaluate individual treatment programmes based on a sound knowledge of evidence based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment skills.
- To comply and uphold standards of the HCPC and the Royal College of Occupational Therapists as indicated by Professional registration
- To maintain own continuing professional development (CPD) by keeping abreast of any new trends and developments, and incorporate them as necessary into your work.
- To be an active member of the in-service training programme by attending, feeding back and delivering presentations and training sessions at staff meetings and tutorials, and by attending external courses and using reflective practice.
- To communicate effectively and work collaboratively with medical, nursing and therapy colleagues to ensure delivery of a coordinated multidisciplinary service. This will include case conferences, patient reviews and discharge planning.
- To participate in the staff appraisal scheme and Personal Development Plan (PDP) as appraisee and appraiser.

Essential criteria
- OT degree or equivalent professional qualification
- UK HCPC registration
- Evidence of post registration CPD relevant to Older People
Desirable criteria
- Membership of professional organization
Qualifications & Knowledge
Essential criteria
- Relevant post registration experience in neurology and older peoples services.
- Previous previous experience in musculoskeletal/neurology/respiratory/ frailty/ orthopaedics
- Previous experience of working in the NHS / awareness of NHS policy / falls guidance
Desirable criteria
- Experience of supervision of students/ support workers
- Experience of providing falls prevention education / classes

- UK Car Driver and provision of car
