Neonatal Community Outreach Senior Sister

Job overview

The post holder is responsible for leading the Neonatal Community Outreach Team, the assessment of care needs, the development, implementation and evaluation of programmes of care without supervision in both the hospital and the community setting.   The post holder will work across site in both hospitals and Community environments when required.

  The post holder is required to teach and supervise families and other nursing and non-nursing staff and be a resource person within both units for discharge planning and outreach support.

 To be a professional role model, leading and co-ordinating the continuation of care in the community/home stay of babies with on-going special needs. Taking continuing responsibility and accountability for the management and organisation of the community team. This involves managing a wide range of complex and sometimes conflicting priorities

 At all times ensure that the needs of the baby are placed at the centre of care delivery, monitoring standards and taking action to maintain and improve them.

 Manage  appraisals and appropriate professional development Identify and manage performance issues in line with Trust policies.

Main duties of the job

 To provide sound clinical leadership and develop support mechanisms for sharing good practice within the clinical setting and Trust wide.   

 

Maintain and promote effective communication both verbally and in writing with all members of the multi-disciplinary team and other Wards or Departments, including formal and informal presentation of information relating to your service.  Liaise with staff in other departments to ensure that the specific needs and care of the baby and his/her family are identified and met.

 

To demonstrate politeness, courtesy and sensitivity in dealing with patients/clients, visitors/relatives and colleagues, maintaining good customer relations.

Detailed job description and main responsibilities

To make assessment of the baby’s physical, psychological, social and spiritual needs, without direct supervision. Recognises the significance of any change in the baby’s condition.

 

Actively plans care to meet the needs identified, and using evidence-based practice, adapting and updating the plan of care where appropriate, utilising available resources efficiently, without direct supervision.

 

Carry out skilled nursing care and evaluates the effectiveness of this, providing clear and concise documentation, maintaining a safe and therapeutic environment for babies and carers. 

 

Provides family centred care and provides information, teaching and support to families to enable them to care for the infant themselves wherever possible, both in hospital and the community.

 

Able to manage and take continuing responsibility for a given caseload in the community: planning, prioritising and delegating work.

 

To fully prepare babies requiring home oxygen therapy and their families for home, according to the Paediatric Pan London Oxygen Group guidance

 Monitor patients’ and parents’ progress, work towards safe and timely discharge plans and ensure barriers to discharge are identified and acted on appropriately.

Essential criteria
  • Registered Nurse (Child/Adult)
  • Post registration qualification relevant to area of speciality or equivalent Neonatal intensive care qualification
  • Recognised Mentorship/ Teaching qualification
  • Neonatal Community Module/ Qualification in community Nursing
  • Willingness to undertake Neonatal Community module
  • Neonatal Community Module/ Qualification in community Nursing
  • Experience working within Community Team
  • Relevant Post registration experience working as an experienced Band 6
  • Experience of excellence in leading all/part of a clinical team in practice
  • Proven management experience
  • Proven clinical knowledge linked to area of speciality and excellence in care delivery
  • Proven knowledge of safety issues through clinical governance and risk management
  • Excellent communication skills with patients, relatives, staff and external contracts including in challenging situations, demonstrating respect and kindness
  • Experience of data collection for quality monitoring/audit
  • Experience of risk management/assessment
  • Experience of using databases
Desirable criteria
  • Involvement in clinical audits and clinical governance
  • Master’s degree or working towards one
  • Experience of clinical supervision
