Job overview
As part of the Trust’s commitment to Personalised Care, RMH is implementing an OAFU (Open Access Follow up) model for Testicular and Prostate cancer patients called the Empower Pathway.
The Trust is recruiting a GP for four sessions a week (session is 4 hours) to help set up and run these pathways. The role will suit a GP with a keen interest in gaining further experience in the field of urological cancers. Pathways will be led by a GP/Advanced Nurse Practitioner team; also working with two CNS’, a Pathway Co-ordinator as well as the wider urological team.
Main duties of the job
- Enable and facilitate patients to self-manage (educating patients on long-term physical and mental health, developing a monthly education programme to manage residual symptoms)
- Deliver a personalised holistic care-plan (including the development of a personalised care plan/information pack) to patients following cancer treatment, improving patient experience
- Ensure post-treatment support relating to side effects of treatment is holistic- including psychosocial considerations. This may require the involvement of other specialist colleagues when required.
- To work on the continuing development of these pathways with surgical and Oncological consultants.

Qualifications
Essential criteria
- MB BS or equivalent
- GMC registration and entry on the General Practitioner register
- DBS clearance
- Attendance at healthcare management Course or Management qualification
Experience
Essential criteria
- Demonstrable experience of urological cancers (2 years or more)
- Ability to translate and integrate evidence-based research/guidelines into the development of local pathways of care
- Demonstrate understanding of current health environment and impact of current government/regional policy developments
- Ability to lead within multiprofessional working within an organisation and across organisational boundaries
- Demonstrate leading innovative practice development and evidence of significant achievements gained
- Previous experience of formal presentations to multi professional group
Desirable criteria
- Evidence of commitment to education and training e.g. educational supervisor, courses and/or setting up an educational programme
- Evidence of publications as both primary and secondary author
- Evidence of managing clinical services, service innovation and up to date knowledge of medical policy, strategy together with successful change management experience and a proven track record of launching new initiatives
- Evidence of project management
